THIRD PARTY LOGISTICS PROVIDER
A third party logistics provider (3PL) contracts with a prescription drug wholesale distributor or prescription drug manufacturer to provide warehousing, distribution or other logistics services on behalf of a manufacturer or wholesale distributor, but does not take title to the prescription drug or have responsibility to direct the sale or disposition of the prescription drug.
Application: Businesses seeking this permit must complete the Department’s application. Mail or deliver the application and all required attachments to:

Other Prescription Drug Licenses and Permits: Submit a legible photocopy of all permits and licenses issued to the applicant’s address authorizing the purchase of prescription drugs at that address, regardless of the issuing agency – state, federal or otherwise.
Contracts: Applicants and permittees must maintain contractual relationships with the prescription drug wholesale distributors and/or manufacturers for whom the applicant or permittee provides 3PL services.
Customer List: Applicants and permittees must disclose to the Department any and all prescription drug wholesale distributors and/or manufacturers for whom the applicant or permittee provides 3PL services.
License Term: Third Party Logistics Provider permits are valid through the last day of the initial issuance month in the second year of licensure. Example: A permit issued on September 12, 2011 would be valid through September 30, 2013.
Inspection: Passing an onsite inspection is a prerequisite to issuance of this permit to establishments located in Florida, although the Department may conduct a pre-permit or routine inspection, onsite or otherwise, for out-of-state applicants.
Fees: The application and permitting fee is $600 for a two-year permit, plus a one-time pre-permit inspection fee of $150 for establishments in Florida. Make checks payable to DBPR. The inspection fee applies only to new permits issued to establishments in Florida.
