Slot Machine Business Entity Occupational License Application fee is $1,000 for a one-year license, $2000 for a three-year license. Payment can be made by check or money order made payable to DBPR. Occupations include (but are not limited to): slot machine management companies, service companies, vendors, distributors, manufacturers, testing laboratories. Required Forms: DBPR PMW-3420 – Slot Machine Business Entity Occupational License Application DBPR PMW 3460 – Authorization for Release of Information Applications for a Slot Machine Business Entity Occupational License may be completed online or mailed to: DBPR – Division of Pari-Mutuel Wagering Licensing Section 2601 Blair Stone Road Tallahassee, FL 32399-1037 All Officers, Directors, and Shareholders of 5% or more interest in the business entity who do not need access to a slot machine facility in Florida, must submit a fingerprint card and the $37.25 fingerprint processing fee upon submission of the Slot Machine Business Entity Occupational License Application. An individual Authorization for Release of Information must be completed. Any Officer, Director, or Shareholder of 5% or more interest of a business entity who needs access to a slot machine facility in Florida, must obtain a Slot Machine Business Employee Occupational License. The fingerprint fee is $37.25 the first year and $13.25 every 3 years thereafter. A fingerprint card may be obtained at any Pari-Mutuel facility; your local law enforcement office (prints must be taken on an “Applicant” fingerprint card); by contacting the division at the number listed below; or you may submit your fingerprints electronically through a law enforcement agency or an approved vendor that has a live scan or hard card scanning device. If you are able to submit your fingerprints electronically, please submit them with the following required ORI number: FL923230Z. Fingerprints will be electronically retained for future submissions to the FDLE if there is no lapse in licensure. Occupations include (but are not limited to): slot operations managers; slot shift managers; floor supervisors; slot tech managers; slot tech supervisors; slot technicians; slot attendants; facility (slot machine licensee) management; slot machine officers, directors and shareholders; chiefs of security; security officers; surveillance directors, investigators, supervisors, operators, and technicians; maintenance supervisors; controllers; count team employees; cage cashiers; count room managers and supervisors; cage managers and supervisors; information systems managers; systems analyst supervisors; operations analyst supervisors; revenue audit managers and supervisors. Required Forms: DBPR PMW 3410 – Individual Slot Machine Occupational License Application DBPR PMW 3460 – Authorization for Release of Information Two forms of identification as required by Rule 61D-14.010, Florida Administrative Code. Slot Machine Business Employee Occupational License For general- and professional-level employees who work for a business entity and need access to a slot machine facility in Florida. Application fee is $50 for a one-year license, $100 for a three-year license, plus a $37.25 fingerprint processing fee. Payment can be made by check or money order made payable to DBPR. Occupations include (but are not limited to): all occupations in the professional and general employee professions listed above. Slot Machine “Combo” Professional Individual License: For anyone who works in a professional-level position and needs access to the slot machine area and other restricted areas of the pari-mutuel facility to perform his/her duties. For example, a slot machine technician who is also a card dealer should apply for a slots combo professional license. Application and fingerprint fees must be submitted to the slot machine licensee (permit holder). Occupations include (but are not limited to): all professional-level slots, pari-mutuel, and cardroom license occupations. Required Forms: DBPR PMW 3410 – Individual Slot Machine Occupational License Application DBPR PMW 3460 – Authorization for Release of Information Two forms of identification as required by Rule 61D-14.010, Florida Administrative Code. Special Note: Pursuant to s. 205.171, F. S. a disabled, honorably discharged veteran from one of the time periods specified in s.1.01(14), F.S., or the unremarried spouse of a deceased disabled veteran meeting this criteria, may be exempt from license fees up to $50. Fingerprints are required just like any other unrestricted license. Fingerprint fees are not exempt.
