Activity Reports
The Division of Hotels and Restaurants conducts regular inspections of public food service establishments to assure compliance with state sanitation and safety laws. Those establishments that do not comply with the laws are subject, according to Florida law, to administrative disciplinary actions which can include license revocation, suspension or fine. The published information is:
Case Number consists of the Division of Hotels and Restaurants district number, the year of the case and a sequential case number.
License Number consists of a three-digit code denoting restaurant type (usually SEA for seating or NOS for nonseating), the county number and the official license number (sequential by county). You can use the license number to look up license or inspection information on DBPR Online Services.
Business Name and Address is the business location. All addresses are Florida since we only regulate businesses located in the State of Florida.
Items is the combined total number of critical and non-critical violations cited on the original inspection that led to the compliance action.
Fine is the amount of the fine assessed on the final order.
Final Order Signature Date is the date the order was clerked by the Agency Clerk and therefore finalized by the division.
Incident Date is the date of the original inspection that resulted in the case. You can look this up on DBPR Online Services. Some of the older reports do not have this column.
Cases are listed in Restaurant Disciplinary Activity Reports according to the month the director signed the final order. Within a report they are listed by district and the chronological case number.
