Delinquent Account Reporting form

Used by licensed distributors to notify the Division that a licensed vendor has not paid  for alcoholic beverage purchases within the time prescribed by law.

Once your delinquent form(s) have been completed and saved, a copy of the form(s) should be sent to the following email address.
