ATTENTION INSPECTORS:
Inspection Reports Must Include 6-Digit Serial NumberEffective immediately, inspectors must write a number and fill-in the bubble for all six digits in the serial number section of the Elevator Inspection Report. The Bureau recently began issuing serial numbers above 100000, expanding the serial number from five to six digits. Serial numbers with fewer than six digits should have leading zero's added. For example, serial # 123 should be entered as 000123. Inspection reports submitted without all six digits entered will be returned for correction.
Pit Shut Off Valve Correction
Effective July 5, 2012, any elevator containing hydraulic supply shut off valves, also known as "knock-style valves", are in violation of the Florida Elevator Safety Code. Certified elevator inspectors must cite the violation and the violation must be corrected to obtain a current satisfactory inspection. In order to comply with the safety requirements adopted by the division, the only acceptable method of correction is to replace the valve.
The Bureau of Elevator Safety previously published Industry Bulletin 2010-01 on January 11, 2010, regarding the dangers of hydraulic supply shutoff valves.
Two-Stop Exemption Notifications
The Bureau of Elevator Safety is updating the service maintenance contract information for two-stop elevators that are exempt from the annual inspection requirement. To continue qualifying for the exemption, you must notify the bureau of your service maintenance contract by submitting a statement that you have a full-service maintenance contract with a registered elevator company that is performing all required maintenance and testing. A copy of a bill statement for a service maintenance contract does not qualify you for the exemption.
Direct Supervision
Rule 61C-5.008, Florida Administrative Code –Definitions
(7) “Direct supervision” means a certificate of competency holder supervising an elevator helper as set forth in section 399.01(16), Florida Statutes, is within physical proximity of the person being directly supervised such that timely observation and evaluation of the work being performed is facilitated to ensure the work is completed in accordance with applicable statutory and administrative code requirements.Initial Inspection
Section 399.03(5), FS As required in Florida Statute for new installations, when an initial inspection is conducted, it must be submitted in hardcopy to the bureau along with the Supervisor of Construction signature and the affidavit of code compliance.
