| Due to Governor Crist's Plain Language Initiative, the Division of Hotels and Restaurants has updated its application packets and forms. Please make sure you are using the current forms to ensure faster processing. |
APPLICATION PACKETS CONTAIN ALL INSTRUCTIONS AND FORMS NEEDED TO COMPLETE THE PROCESS. Please mail the application and fees to the Tallahassee address on the application packet. Once you have completed the plan review and construction of the facility, and submitted the application forms and fees, call the Customer Contact Center at 850.487.1395 to request an "opening" inspection. An inspector in the H&R district office where the facility is located will contact you to schedule the inspection. To make sure that you are able to open your establishment in a timely manner, please call a minimum of one week prior to the desired inspection/opening date.
