Plan Review Forms
For seating restaurants; take-out/delivery or food courts (nonseating); caterers; mobile food dispensing vehicles; hot dog carts; and theme park food carts.
Plan review is required for food service establishments that are:

Newly built,
-
Converted from another use,
-
Remodeled or
- Re-opened after being closed at least 1 year.
If you are not sure if your food service establishment needs a plan review, please call the Customer Contact Center at 850.487.1395. Additional information is available on our plan review web site. Also, see our guidelines for planning your food service operation.
Plan review applicants may be eligible to complete the plan review process electronically. This option is available to anyone with the technological ability to submit high-quality plans drawn to scale, including architectural or engineering plans or blueprints. If you are interested in completing the plan review process electronically, please contact the plan review office at dhr.planreview@dbpr.state.fl.us.
After your plans are approved, you still must be approved for a license. See our licensing web site for more information.
