PARI-MUTUEL WAGERING PERMITHOLDER INFORMATION

The application and attachments are to be completed by individuals or businesses desiring to obtain a Permit to Conduct Pari-Mutuel Wagering.  Upon completion, the original notarized application and all attachments, with a Cashier’s Check for $15,000, made payable to the Department of Business and Professional Regulation, should be returned to the:

DBPR PMW-3195 Request for Release of Information
DBPR PMW-3030 Personal History Record
DBPR PMW-3120 Individual Occupational License Application (for each officer and director)
DBPR PMW-3190 Officers and Directors

Permit Investigation Deposit ------------------------------- $15,000
Deposit not required for a quarter horse or non-wagering permit application.
