Florida Slot Occupational Licenses are valid from July 1st through June 30th of the following year.

Slot Machine License (for permitholders):
Application fee = $3,000,000 and Regulatory fee = $250,000 (must be submitted via EFT).
Required Forms: DBPR PMW 3460 – Authorization for Release of Information See Chapter 551.107, Florida Statutes, for further details on required documents for obtaining a slot machine license.

Slot Machine Business Entity Occupational License
Application fee is $1,000 for a one-year license, $2000 for a three-year license. Payment can be made by check or money order made payable to DBPR.
Occupations include (but are not limited to): slot machine management companies, service companies, vendors, distributors, manufacturers, testing laboratories.
Required Forms: DBPR PMW 3435 – Affidavit of Truth (Please note: DBPR PMW-3430 & DBPR PMW-3435 should not be submitted to the Division. Instead, these forms must be maintained at an office of the business entity in Florida, or with the company’s registered agent in Florida pursuant to Rule 61D-14.060(3).)
Applications for a Slot Machine Business Entity Occupational License must be submitted to the following address: DBPR – Division of Pari-Mutuel Wagering
All Officers, Directors, and Shareholders of 5% or more interest in the business entity who do not need access to a slot machine facility in Florida, must submit a fingerprint card and the $43.25 fingerprint processing fee upon submission of the Slot Machine Business Entity Occupational License Application.
Any Officer, Director, or Shareholder of 5% or more interest of a business entity who needs access to a slot machine facility in Florida, must obtain a Slot Machine Business Employee Occupational License.
Any business employee who needs access to a slot machine facility in Florida must obtain a Slot Machine Business Employee Occupational License. |

Slot Machine Professional Employee Occupational License
Application fees and fingerprint fees must be submitted to the slot machine licensee (permitholder).
Occupations include (but are not limited to): slot operations managers; slot shift managers; floor supervisors; slot tech managers; slot tech supervisors; slot technicians; slot attendants; facility (slot machine licensee) management; slot machine officers, directors and shareholders; chiefs of security; security officers; surveillance directors, investigators, supervisors, operators, and technicians; maintenance supervisors; controllers; count team employees; cage cashiers; count room managers and supervisors; cage managers and supervisors; information systems managers; systems analyst supervisors; operations analyst supervisors; revenue audit managers and supervisors.

Applications for a Slot Machine Professional Employee Occupational License must be submitted at the State Office where the employee is working. |

Slot Machine General Employee Occupational License
Application fees and fingerprint fees must be paid by the slot machine licensee (permitholder).
Occupations include (but are not limited to): maintenance employees; information systems technicians; systems and operations analysts; revenue auditors; income audit analysts and auditors; and food service employees.

Slot Machine Business Employee Occupational License
For general- and professional-level employees who work for a business entity and need access to a slot machine facility in Florida. Application fee is $50 for a one-year license, $100 for a three- year license, plus a $43.25 fingerprint processing fee. Payment can be made by check or money order made payable to DBPR.
Occupations include (but are not limited to): all occupations in the professional and general employee professions listed above.

Slot Machine Business Officer, Director, Shareholder: No license issued. This profession is for Officers, Directors, and Shareholders of a slot machine business entity who do not need access to a slot machine facility in Florida. The fingerprint processing fee is $43.25. Payment can be made by check or money order made payable to DBPR.

| 1055 | Slot Machine “Combo” Professional Individual License: For anyone who works in a professional-level position and needs access to the slot machine area and other restricted areas of the pari-mutuel facility to perform his/her duties. For example, a slot machine technician who is also a card dealer should apply for a slots combo professional license. Application and fingerprint fees must be submitted to the slot machine licensee (permitholder). Occupations include (but are not limited to): all professional-level slots, pari-mutuel, and cardroom license occupations.
Required Forms: DBPR PMW 3410 – Individual Slot Machine Occupational License Application DBPR PMW 3460 – Authorization for Release of Information DBPR PMW 3440 – Professional Occupational Supplemental Information (please note: this form should not be submitted to the Division. Instead, this form must be kept on file at the slot machine facility). Two forms of identification as required by Rule 14.010, Florida Administrative Code. Fingerprints (must be taken at the slot machine facility). Fingerprints are due upon initial licensure and every three years thereafter.
