Report Unlicensed Activity or File a Complaint

DIRECTIONS FOR FILING A COMPLAINT:

Select the professional category from the list below by clicking on the name of the profession. You may either type in your information, print the completed form and sign OR print the blank form, type/write in your information and sign. The completed form cannot be saved to your PC.

In order to make a legal evaluation, any and all supporting documentation regarding the allegations must be in the department's possession. The complaint must also be signed by the complainant where indicated. In most cases, if supporting documentation and the signed complaint are not received within 30 days, the complaint will be dismissed.

File your complaint by:

• mailing the completed form and supporting documents to the DBPR address on the last page of the complaint form.
• emailing the completed form and supporting documents to ULA@dbpr.state.fl.us.

*Note: Florida law provides that any information contained in a complaint against an unlicensed person is public record.
