This application is used by licensees to request a duplicate wall certificate that has been either lost or damaged.

WALL CERTIFICATE: Return department-issued wall certificate or submit a notarized statement indicating why the original wall certificate cannot be returned. Wall certificates must be mailed to: Division of Certified Public Accounting, 240 N.W. 76th Drive Suite A, Gainesville, FL. 32607
