Cardroom Employee Occupational License - Temporary

All cardroom employees except food service, security, maintenance, or mutuel teller employees. Possession of this license does not allow access to any restricted areas other than the cardroom.

Application Requirements:
 
  • APPLICATION: Individuals who have been previously licensed by the division may complete a DBPR PMW-3120 Individual Occupational License Application from. Complete the application by clicking on the "Printable Application" link at the bottom of the page. Please hand-deliver or mail your application to the pari-mutuel facility where you will be conducting business attn: State Office. To locate the address of the facility closest to you please review the facility list provided.

  • LICENSE TERM: Florida Pari-Mutuel Licenses are valid from May 1st through June 30th of the third year.

  • FEE: License fee $120. For a three-year license. (make check payable to DBPR). An honorably discharged disabled veteran from one of the time periods specified in s.1.01(14), F.S., or the unremarried spouse of a deceased disabled veteran meeting this criteria, may be exempt from license fees up to $50.00. Fingerprint fees are not exempt.

  • FINGERPRINTS: Applicants must submit fingerprint cards and pay $36.00 fingerprint fee the first year and every 3 years thereafter. If fingerprints are due at renewal; the applicant must pay a $12.00 for his/her to be resubmitted to the Florida Dept. of Law Enforcement for an updated criminal history background check.

    You may submit your fingerprints electronically through a law enforcement agency that has a live scan or hard card scanning device. If you are able to submit your fingerprints electronically, please submit them with the following required ORI number: FL920630Z.

    Fingerprints will be electronically retained for future submissions to the FDLE. Fingerprints can be mailed to DBPR; Division of Pari-Mutuel Wagering; Licensing Section, 2601 Blair Stone Road, Tallahassee, FL 32399-1037.
