Slot Machine/Pari-Mutuel/Cardroom Combo Professional Individual - Renewal

For anyone who works in a professional-level position and needs access to the slot machine area and other restricted areas of the pari-mutuel facility to perform his/her duties. For example, a slot machine technician who is also a card dealer should apply for a slots combo professional license. Occupations include (but are not limited to): all professional-level slots, pari-mutuel, and cardroom license occupations.

Application Requirements:
 
  • APPLICATION: A DBPR PMW-3415 Individual Slot Machine Occupational License Application form may only be used by individuals with a valid license that expires on June 30, 2010. Complete the application by clicking on th "Printable Application" link at the bottom of the page. Application fees and fingerprint fees for Slot Machine General and Slot Machine Professional Employees must be submitted to the slot machine licensee (permitholder). Please hand-deliver or mail your application to the pari-mutuel facility where you will be working. To locate the address of the slot machine facility closest to you please review the facility list provided.

  • LICENSE TERM: Florida Slot Machine Licenses are valid from May 1st through June 30th of the following year.
  • FEE: Application fee is $50 for a one-year license, $100 for a three-year license, plus a $16.50 fingerprint resubmission (processing) fee. Payment can be made by check or money order made payable to DBPR. Application and fingerprint fees must be paid by the slot machine licensee (permitholder). An honorably discharged disabled veteran from one of the time periods specified in s.1.01(14), F.S., or the unremarried spouse of a deceased disabled veteran meeting this criteria, may be exempt from license fees up to $50.00. Fingerprint fees are not exempt.
  • FINGERPRINTS: Applicants must be fingerprinted at the slot machine facility. Fingerprints are due upon initial licensure and every three years thereafter. If fingerprints are due at renewal; return to submenu and complete an Initial Application.
