Request Main Office Duplicate License (Form RE 12)

This transaction is used to allow the broker to obtain a duplicate copy of the Main Company license when the previous license has been lost, stolen, or destroyed.

Application Requirements:
  • APPLICATION: Complete the appropriate section(s) of the application by clicking on the "Printable Application" link at the bottom of the page.

    A duplicate license can only be issued if the license is in a Current/Valid status. We are unable to issue a duplicate license on an expired license.

  • FEE: The fee is $25 which can be paid by personal check or money order.
