Appraisal Management Company Maintenance (FREAB 2)

The Appraisal Management Company maintenance form may be used to update the companies addresses, name, request a duplicate license, close a company, or renew a license.

Application Requirements:
 
  • Close AMC: If an Appraisal Management Company no longer wishes to operate in the state of Florida they may complete section II of the form. The signature would have to be of an Authorized Representative or Registered Agent that has signature authority. There is no fee associated with this transaction.
  • Change of Address (no fee): Section III of the form can be used to change the mailing address and/or physical location of the Appraisal Management Company. The section should be signed by an Authorized Representative or Registered Agent with signature authority. There is no fee to complete this transation.
  • Change of Address (fee): Section IV of the form allows the Appraisal Management Company to change their mailing address and/or phsyical location. In addition, they may request a new license with the updated information for a $20.00 fee. This section is to be signed by the Authorized Representative or Registered Agent with signature authority.
  • Request a Duplicate License: Section V of the form is to be used when the Appraisal Management Company requires a duplicate copy of their license due to the previous license being destroyed, lost, change of address, or change of name. This section will require a signature of the Authorized Representative or Registered Agent with signature authority. Please include a fee in the amount of $20.00 with your request.
  • Name Change: Section VI of the form may be used to change the registered name or doing business as name of an Appraisal Management Company. Prior to the name change being completed, the name has to be registered or change with the Department of State, Division of Corporations. Please submit a $20.00 fee with this request. This section is to be signed by the Authorized Representative or Registered Agent with signature authority.
  • Renewal: Section VII of the form may be used by the Appraisal Management Company should they need to renew the license and did not receive a renewal notice; the notice has been destroyed; pr the notice has been lost. Please refer to our website for current renewal fee information. If the renewal is being paid after the expiration date, please include an additional $25.00 late fee with the renewal payment. This section is to be signed by the Authorized Representative or Registered Agent with signature authority.
