Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

24-08-4    Basic - Equipment and utensils not properly air-dried - wet nesting. Several clean dishware items wet nesting at drying rack.
33-19-4    Basic - Garbage on the ground and/or pad around dumpster. Garbage on ground around shared dumpster under dumpster with flying insects present.
35B-03-4    Basic - Outer openings not protected with self-closing doors. Self closer disconnected from back door.
36-02-5    Basic - Unsealed concrete floor in food preparation, food storage, warewashing area or bathroom. Kitchen floor not smooth and easily cleanable. Observed debris building on floor along walls.
01B-01-4    High Priority - Dented/rusted cans present. See stop sale. Six cans of pineapple with dents along seam or sharp dents.
35A-02-7    High Priority - Live, small flying insects found. Observed two flying insects in establishment. One at front counter and one at back smoothie area.
22-43-4    High Priority - Quaternary ammonium sanitizer not at proper minimum strength for manual warewashing. Do not use equipment/utensils not properly sanitized. Quatanery ammonium at triple sink testing at 0ppm even after changing multiple times. Person in charge will hand mix until dispenser can be serviced. **Corrective Action Taken**
22-02-4    Intermediate - Food-contact surface soiled with food debris, mold-like substance or slime. Green cutting boards in clean dishware area with debris buildup, staining.
