Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

22-20-5    Basic - Accumulation of black/green mold-like substance in the interior of the ice machine/bin. Observed accumulation of mold like substance inside the ice ,machine used for drinks.
16-03-4    Basic - Accumulation of debris inside warewashing machine. Observed mold like substance inside the dishwasher located on the first floor.
14-01-5    Basic - Bowl or other container with no handle used to dispense food. Observed plastic container without handle to scoop the jark sauce at the Manjay location. **Repeat Violation**
14-09-4    Basic - Cutting board has cut marks and is no longer cleanable. Observed excessive amount of cut marks on the long white cutting board.
12B-07-4    Basic - Employee beverage container on a food preparation table or over/next to clean equipment/utensils. Observed employee open cup with coffee on prep table. **Repeat Violation**
13-03-4    Basic - Employee with no hair restraint while engaging in food preparation. Observed owner of Shores , handling food without hair restraint.
10-20-4    Basic - In-use tongs stored on equipment door handle between uses. Observed tongs on oven handles in Pho sho venue. Employee removed the tongs. **Corrected On-Site**
21-04-4    Basic - In-use wiping cloth/towel used under cutting board. Observed towel underneath cutting board at the USBS.
05-09-4    Basic - No conspicuously located ambient air temperature thermometer in holding unit. RIC located at the second d floor.
23-03-4    Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed mold like substance on reach in cooler gaskets 2nd floor.
42-01-4    Basic - Wet mop not stored in a manner to allow the mop to dry.
12A-12-4    High Priority - Employee switched from working with raw food to be touching clean containers and utensils without washing hands. Observed owner of Shores venue touching the fish with bare hands unnecessarily. Owner was coached and explained about it. He washed his hands and placed gloves on. **Corrected On-Site**
03A-02-5    High Priority - Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed feta cheese at 49 F inside reach in cooler for less than 4 hrs , RIC was at proper temperature of 37 F). Employee placed ice underneath. **Corrective Action Taken**
01C-05-4    Intermediate - Clam/mussel/oyster tags not maintained in chronological order according to the last date they were served in the establishment. Observed oyster tags not organized in chronological order. Observed at the Shores.
09-17-4    Intermediate - Employee using bare hand contact with nonready-to-eat food (such as raw animal food or flour) unnecessarily without the use of gloves, deli paper, scoops, tongs or other utensils. Observed owner of Shores venue touching the fish with bare hands unnecessarily. Owner was coached and explained about it. He washed his hands and placed gloves on. **Corrected On-Site**
31A-09-4    Intermediate - Handwash sink not accessible for employee use at all times. Observed some metal containers blocking the access to the hand sink located at the ware washing area, first floor. Employee moved all of them immediately. **Corrected On-Site** **Repeat Violation**
02D-12-1    Intermediate - Juice packaged in the establishment that has not been specifically processed to prevent, reduce, or eliminate the presence of pathogens not bearing a warning label. Observed missing labels for fresh veggie juices that have been processed on site and packaged inside sealed plastic bottles in advance to customer orders. Juices have not been pasteurized. Person in charge discarded them . GM stated they will not be exposed and sold in the same way until they have the correct labels. **Corrected On-Site**
03F-10-5    Intermediate - No written procedures available for use of time as a public health control to hold time/temperature control for safety food. Observed missing TPHC procedure on place for Maki Son and Poke Son . Procedure was provided and filled it out. **Corrected On-Site**
41-17-4    Intermediate - Spray bottle containing toxic substance not labeled. Missing label for spray bottle with chemical product ( degreaser ) . Employee labeled the bottle. **Corrected On-Site**
