Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

32-23-4    Basic - Toilet/urinal not flushing/functioning properly. Women's restroom toilet not functioning. The operator had a sign up stating restroom was out of order.

14-41-4    Basic - Walk-in cooler gasket torn/in disrepair. **Repeat Violation**
36-26-4    Basic - Wall soiled with accumulated black debris in dishwashing area. Under drain board near dish machine. **Repeat Violation**
29-11-4    Basic - Water leaking from pipe and/or faucet/handle. Apt sink on bar area. Pipe under three compartment sink leaks. **Repeat Violation**

35A-05-4    High Priority - Roach activity present as evidenced by live roaches found. One live roach found on caster of crash cart on cook line. The operator showed me a recent pest control invoice dated 2/18/16. Pest control showed up during the inspection. **Warning**

29-37-4    High Priority - Spray hose at dish sink lower than flood rim of sink. Red hose hanging in sink **Repeat Violation**
22-25-4    Intermediate - Accumulation of encrusted food debris on/around lentil grinder. Water, towel and food mixture in bottom of lentil grinder in storage room.
22-22-4    Intermediate - Encrusted material on can opener blade. **Corrected On-Site**
14-14-4    Intermediate - Nonfood-grade basting brush used in food.
03D-19-4    Intermediate - Potentially hazardous (time/temperature control for safety) food left at room temperature to cool. Cooked chicken 110°F on counter in kitchen. The operator moved to the walk in cooler. **Corrective Action Taken**
02C-02-4    Intermediate - Ready-to-eat, potentially hazardous (time/temperature control for safety) food prepared onsite and held more than 24 hours not properly date marked. Cooked cauliflower and vegetables in reach in glass door cooler. Cooked chicken and goat in walk in cooler. **Repeat Violation**
