Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

12B-12-4    Basic - Employee beverage container in a food preparation/clean dishware area does not have a lid and a straw or equivalent dispensing mechanism to prevent hand contact with lip contact area of drink container. Open container of soda on prep table in kitchen. **Repeat Violation**
13-03-4    Basic - Employee with no hair restraint while engaging in food preparation.
14-11-4    Basic - Equipment in poor repair. Ice scoop broken.
08B-12-4    Basic - Food stored in holding unit not covered. Sauces and lettuce in walk in cooler.
08B-38-4    Basic - Food stored on floor. Plastic sauces on floor in walk in cooler. **Repeat Violation**
10-17-4    Basic - In-use knife/knives stored in cracks between pieces of equipment. Knife on front line.
10-07-4    Basic - In-use utensil stored in standing water less than 135 degrees Fahrenheit. On front line.
14-31-4    Basic - Nonfood-grade bags used in direct contact with food. Plastic to go bags used to hold chicken in walk in freezer. **Repeat Violation**
06-05-4    Basic - Potentially hazardous (time/temperature control for safety) food thawed in standing water. Raw shrimp in prep sink. I had the operator run cool water on the shrimp to drain. **Warning**
25-05-4    Basic - Single-service articles improperly stored. Boxes of yo go boxes on floor in hall area.
25-06-4    Basic - Single-service articles not stored inverted or protected from contamination. Plastic utensils at self serve stand.
33-34-4    Basic - Storage area not maintained clean and organized. Back hall area single service items and unused equipment stored in disarray.
23-12-4    Basic - Walk-in cooler and/or walk-in freezer shelves soiled with encrusted food debris. Shelves if dry goods near ice machine.
21-12-4    Basic - Wet wiping cloth not stored in sanitizing solution between uses. On cook line.
02D-01-5    Basic - Working containers of food removed from original container not identified by common name. Bulk containers of flour.
41-05-4    High Priority - Pesticide/insecticide labeled for household use only present in establishment. Bottle of hot shot in storage room. **Repeat Violation**
03A-02-4    High Priority - Potentially hazardous (time/temperature control for safety) food cold held at greater than 41 degrees Fahrenheit. Raw chicken 48°F, tofu 48°F, sliced cabbage 50°F. Items held on ice on front line. Ice level not sufficient to keep items cool. **Repeat Violation** **Warning**
41-08-4    High Priority - Toxic substance/chemical stored by or with clean or in-use utensils. Cans of butane near dish ware in kitchen.
41-02-4    High Priority - Toxic substance/chemical stored by or with food. Two spray bottles of cleaner on rack with food.
22-06-4    Intermediate - Ice bucket on top of dish machine is visibly dirty.

03D-19-4    Intermediate - Potentially hazardous (time/temperature control for safety) food left at room temperature to cool. Rice on kitchen prep table 80°F. The operator moved to the walk in cooler.
02C-02-4    Intermediate - Ready-to-eat, potentially hazardous (time/temperature control for safety) food prepared onsite and held more than 24 hours not properly date marked. Cooked foods on walk in cooler.
