Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

36-36-4    Basic - Ceiling tile missing. In back storage room.
35A-03-4    Basic - Dead roaches on premises. Three dead roaches on wall in storage room off of back kitchen.
35B-01-4    Basic - Exterior door has a gap at the threshold that opens to the outside. Exterior room has gaps at doorway to exit. Screening around soffit/fascia and exhaust fans in this room is not adequate to prevent entrance of pest. **Repeat Violation**
36-73-4    Basic - Floor soiled/has accumulation of debris. Near upright chest freezer on cook line.
08B-43-4    Basic - Food stored in a room/shed that is not fully enclosed. Room off of kitchen has open fascia/ soffit. The operator screened the opening, yet some of the screening is not remaining in place. **Repeat Violation**
08B-38-4    Basic - Food stored on floor. Box of limes and potatoes in food storage/ prep room off of kitchen. In walk in cooler sauces stored in floor. **Repeat Violation**
33-23-4    Basic - Grease receptacle/dumpster not on proper pad/nonabsorbent surface. **Repeat Violation**
36-24-5    Basic - Hole in or other damage to wall. In back storage room. Hole in wall under three compartment sink. **Repeat Violation**
33-16-4    Basic - Open dumpster lid.
25-05-4    Basic - Single-service articles improperly stored. Plastic container of to go containers stored on floor near non functioning deli cooler.
36-02-5    Basic - Unsealed concrete floor in food preparation, food storage, warewashing area or bathroom. In back storage/ food prep room. **Repeat Violation**
29-11-4    Basic - Water leaking from pipe and/or faucet/handle. At prep sink in back storage room.
08A-01-4    High Priority - Raw animal food stored over cooked food. Raw chicken stored over cooked chicken in reach in cooler on cook line. **Repeat Violation**
35A-05-4    High Priority - Roach activity present as evidenced by live roaches found. 16 live roaches found in wood shelving in room off of kitchen. 4 live roaches on water heater in storage room off of back kitchen. One live roach on wall in main kitchen area. The operator killed and discarded this roach.
35A-04-4    High Priority - Rodent activity present as evidenced by rodent droppings found. 10 dry hard rodent droppings found under mop sink in storage/ prep room off of kitchen.
35A-18-4    High Priority - Rodent chew marks on bag of rice in storage/ food prep room. See stop sale. Fur found in sticky trap under mop sink in food storage/ prep room off of kitchen.
01B-13-4    High Priority - Stop Sale issued due to food not being in a wholesome, sound condition. Chew marks on bag of rice. See stop sale.
31A-07-4    Intermediate - Handwash sink missing in warewashing or food preparation area. In back storage/prep room there is no hand wash sink. In This room there is an ice machine, a mop sink, prep sink, food storage, clean food contact equipment storage and a walk in cooler.
16-35-4    Intermediate - No chlorine chemical test kit provided when using chlorine sanitizer at three-compartment sink/warewashing machine. **Repeat Violation**
51-16-5    Intermediate - No plan review submitted and approved - renovations were made or are in progress. Must submit plans, plan review application and payment for plan review to DBPR H and R Plan Review office located at 1940 N Monroe St, Tallahassee, FL 32399-1011. No plans on file. Back room with food storage, ice machine, prep sink, and walk in cooler/ chest freezer has no smooth floors, walls and ceilings. Gaps at door, soffit/fascia allow for entrance of vermin. Multiple 35a violations cited.
02C-02-4    Intermediate - Ready-to-eat, potentially hazardous (time/temperature control for safety) food prepared onsite and held more than 24 hours not properly date marked. Cooked meats, sauces and packaged meals in the walk in cooler. **Repeat Violation**
53B-10-4    Intermediate - Records/documents for required employee training do not contain all of the required information. Operator has international food service executives association incomplete certificates for employee training. The operator completed certificate for R,played during inspection. **Corrected On-Site** **Repeat Violation**
