A summary of the violations found during the inspection are listed below.
The department cites violations of Florida's sanitation and safety laws,
which are based on the standards of U.S. Food and Drug Administration's Food Code.
High Priority violations are those which could contribute directly to a foodborne
illness or injury and include items such as cooking, reheating, cooling and hand-washing.
Intermediate violations are those which, if not addressed, could lead to risk factors
that contribute to foodborne illness or injury. These violations include personnel training,
documentation or record keeping and labeling. Basic violations are those which are considered
best practices to implement. While most establishments correct all violations in a timely
manner (often during the inspection), the division's procedures are designed to compel
compliance with all violations through follow-up visits, administration action or closure
when necessary.
Basic - Accumulation of dead insects, or other pests, in control devices.
Observed 8dead roaches and a dead lizard on a glue trap near mop sink in kitchen. Observed 2 dead roaches on a glue under black and white cooler in kitchen. Observed 2 dead roaches on a glue trap under sushi cooler. Manager instructed to remove traps. **Corrective Action Taken** **Warning**
Basic - Dead roaches on premises. Observed 1 dead roach under handwash sink near preparation sink between cookline and back door, 2 dead roach next to mop sink, 1 dead roach under preparation table behind steam stable, 3 dead roaches in dry storage area under shelves, 2 dead roaches between 2 flip top cooler in cook line, 1 dead roach on top of flip top cooler shelf, 3 dead roaches on floor under cookline, 1 dead roach in shelf next to clean plates in sushi bar, 17 dead roaches at register and under sushi cooler in sushi bar, 1 dead roach on preparation table next to walk in cooler, 1 dead roach in walk in cooler.
Manager instructed to remove roaches and to clean and sanitize the areas. Manager instructed to remove clean plates in sushi bar and wash and sanitize them. **Warning**
Basic - Employee personal food not properly identified and segregated from food to be served to the public.
Observed in Insignia White cooler near sushi bar, masago for sushi that belongs to owner is stored with raw tuna and sauces sold to customers, she removed it. **Corrected On-Site** **Warning**
Basic - Potentially hazardous (time/temperature control for safety) food thawed at room temperature. Observed with cooked pork on preparation table between walk in freezer and cooler. Employee placed it in the walk in cooler. **Corrected On-Site** **Warning**
High Priority - Dented cans present. See stop sale.
Observed with 5- 4lb severely dented cans of Elephant King Brand Stripped Bamboo Shoots and 2- 6lb 6 oz cans of Nature Pro brand of Baby Corn. The sides and seams are dented.
Manager instructed to remove cans. **Warning**
High Priority - Employee touched bare body part and then engaged in food preparation, handled clean equipment or utensils, or touched unwrapped single-service items without washing hands.
Observed cook touch nose and head with hands and then began to prepare foods and put on gloves , he was instructed wash his hands and then put on new gloves,he did so with cold water and returned to preparing food. **Warning**
High Priority - Pesticide labeled for household use only present in establishment.
Observed glue pesticide traps with dead roaches and one with a dead lizard on it, label states it is for household use. Manager instructed to remove traps.
Observed can of Raid Ant and Roach Spray stored near preparation sink in kitchen, manager removed it, corrected on site.

High Priority - Potentially hazardous (time/temperature control for safety) food cold held at greater than 41 degrees Fahrenheit.
Observed with 2lbs cooked rice (54°F - Cold Holding), it has been in the black and white reach in cooler for over 4 hours, see stop sale. **Warning**
High Priority - Presence of insects, rodents, or other pests. Observed live millipede insect in office across from Pepsi cooler, manager removed it. **Corrected On-Site** **Warning**
High Priority - Raw animal food stored over ready-to-eat food.
Observed in Insignia White cooler near sushi bar, raw tuna stored above ready to eat sauces, manager instructed properly store foods and did so. **Corrected On-Site** **Warning**
High Priority - Roach activity present as evidenced by live roaches found.
Observed 1 live roach crawling on wall above mop sink,3 live roach crawling on floor under prep sink near cookline, 1 live roach in dry storage area under shelves near triple sink, 2 live roaches crawling on the side of steam table next to flip top cooler.
Manager instructed to kill and remove roaches and to clean and sanitize areas. **Warning**
High Priority - Stop Sale issued on potentially hazardous (time/temperature control for safety) food due to temperature abuse.
Observed with 2lbs cooked rice (54°F - Cold Holding), it has been in the black and white reach in cooler for over 4 hours, see stop sale.

Intermediate - Cold holding equipment not maintained in good repair. Do not store potentially hazardous (time/temperature control for safety) food in this unit until the unit is repaired.
Observed with flip top cooler next to steam table, ambient air temperature is 57°F, manager turned down temperature dial to begin to reduce the temperature.
Foods in this cooler and the flip top cooler across from stove have been in there under 4 hours and were taken out of the walk in cooler and portioned before placing in these coolers. Manager instructed ice down foods and did so. **Corrective Action Taken** **Warning**
Intermediate - Handwash sink not accessible for employee use due to items stored in the sink. Observed bucket stored in handwash sink near back door, manager removed it. **Corrected On-Site** **Warning**
Intermediate - Hot water not provided/shut off at employee handwash sink. Observed water temperature at 84°F at handwash sink in kitchen, manager adjusted water heater, water temperature is now above 100°F. **Corrected On-Site** **Warning**
Intermediate - No hot running water at three-compartment sink. Observed water temperature at 80°F at triple sink in kitchen, water temperature is now above 100°F. **Corrected On-Site** **Warning**
Intermediate - Required employee training expired for some employees. To order approved program food safety material, call DBPR contracted provider: Florida Restaurant and Lodging Association (SafeStaff) 866-372-7233. Observed some employee training certificates that expired on 02/2017 and 04/2017. **Warning**
