Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

32-07-4    Basic - Bathroom facility not clean. Women's and men's toilets are dirty. The urinal is coated with black debris at the drain. The men's handsink is dirty. The large toilet paper roll is not mounted and is luting on the back of the toilet-the side black and dirty from improper hanging/storage. The restroom trash can is dirty. **Warning**
23-06-4    Basic - Build-up of food debris, dust or dirt on nonfood-contact surface. On broom hanging on storage rack near the hot water tank. Wet mop hanging to dry with heavy food debris built up. Near the hot water heater. Under and around the fryers. Heavy french fry buildup and other debris. **Warning**
23-03-4    Basic - Build-up of grease on interior walls of the mop sink and the floor drain by the fryers. Staff educated that heavy grease should be dumped in a grease container, not the drains. Under and around the fryers. **Warning**
23-25-4    Basic - Build-up of grease/dust/debris on hood area, extinguishing lines and pipes over fryers. Caps off on two of 5 ports that are heavily caked with grease. **Repeat Violation** **Warning**
24-03-4    Basic - Clean equipment and utensils not separated from soiled equipment and utensils at warewashing area. Clean pans on the storage rack across from the three bay with chemicals, rusty equipment, soiled cloth mop head, and soiled dishes on the bottom rack below. Strong potential to splash onto clean equipment above. **Warning**
24-06-4    Basic - Clean utensils or equipment stored on a dirty rack attached to a box storage rack outside the walk in cooler door. **Warning**
29-18-4    Basic - Drain cover broken and missing in front of fryers. Drain cover not on mop sink drain. Employee put mop sink cover on mop sink drain. Drain cover missing on the floor drain in front of the ice machine in the back room. **Warning**
14-11-4    Basic - Equipment in poor repair. Water filters mounted on the wall between the ice machine and the dry storage shelves have a black mold-like line down them and standing water below them. **Warning**
36-22-4    Basic - Floor areas covered with standing water: By the walk in cooler where there are two floor drains. Either very soiled floors at the opening hours or backed up drains prior to inspection. Paper and mop debris in the water. Murky patches of dirty water around the area. Water between the ice machine and dry storage goods. Large white puddle under the entire 5 unit fryer line. **Warning**
36-01-4    Basic - Floor not cleaned when the least amount of food is exposed. The entire kitchen and back room floor. By the dry storage goods, wet and dry paper, used tape. Under soda box racks. **Warning**
33-28-4    Basic - Garbage can located inside establishment that is not continually used is not covered. Can is not bagged, very broken. **Warning**
33-19-4    Basic - Garbage on the ground and/or pad and lawn around dumpster. **Warning**
22-08-4    Basic - Interior of oven has heavy accumulation of black substance/grease/food debris. **Warning**
22-04-4    Basic - Lime scale and debris build-up on the edge of the ice machine where the door contacts the unit over the ice. **Warning**
36-70-4    Basic - Objectionable odor in kitchen. **Warning**
33-16-4    Basic - Open dumpster lid. **Repeat Violation** **Warning**
29-08-4    Basic - Plumbing system in disrepair. Water filters mounted on the wall between the ice machine and the dry storage shelves have a black mold-like line down them and standing water below them. **Warning**
25-05-4    Basic - Single-service articles improperly stored. Commercially plastic bag-wrapped take-out containers on the lowest shelf and splashed from the adjacent dripping filter. Stacks of napkins at the drive thru still held together with the brown paper wrap are observed soiled on the sides. The brown paper and napkins have black marks with improper storage appearance. **Warning**
25-06-4    Basic - Single-service articles not stored inverted or protected from contamination. Stack of large cup lids at the drive thru are soiled with brown soda. Wrapped take out cups stacked on the shelf near the drive thru soda machine. **Warning**
25-17-4    Basic - Single-service articles stored on a soiled surface. Take out box/napkin rack at the drive thru. **Warning**
25-09-4    Basic - Single-service items for customer self-service not properly protected to prevent contamination. Soiled cardboard boxes at the front line holding take-out bags. **Warning**
23-05-4    Basic - Soil residue build-up on nonfood-contact surface. Wet mops hanging to dry with heavy food debris built up. Trash can castor roller with the can removed has heavy dried delight brown debris on it. Build up of flour-chicken debris and other trash. On exterior of food package boxes on storage rack outside the walk in cooler. Behind bun toaster, reach in cooler equipment. Lids, bags, debris, crumbs. On chicken prep cart wheels. Make table cooler unit wheels. Upright unit wheels. On wall mounted fire extinguisher. Interior of both restroom doors. Hinges of hot units, shelves of tables, hinges of coolers. Trash cans. Drive thru cup holder rack mounted on the wall holding unwrapped lids. Register counters. Shelving under the drive thru register at the power strip. Walk in unit doors and handles. **Repeat Violation** **Warning**
23-09-4    Basic - Soiled reach-in cooler gaskets. **Warning**
29-19-4    Basic - Standing water in floor drain/floor drain draining very slowly. In front of the fryer. When washing hands at the hand sink in front of the fryers by the kitchen entrance, it is observed that a thick brown substance rises in the floor drain in front of the fryers. When the hand sink is shut off, the drain contents retreat. **Warning**
36-72-4    Basic - Walk-in cooler and walk-in freezer floors soiled. **Repeat Violation** **Warning**
36-27-5    Basic - Wall soiled with accumulated grease, food debris, and/or dust. On both sides of the walk in cooler door in the kitchen. On alarm box over the reach in cooler. **Warning**
02D-01-5    Basic - Working containers of food removed from original container not identified by common name. Bulk flour bin. **Repeat Violation** **Warning**
36-10-4    Basic - Worn, torn and/or soiled floors/carpeting. Along walls behind equipment is heavy buildup. **Warning**
22-50-4    High Priority - Food-contact surfaces sanitized with a quaternary ammonium solution not exposed to the sanitizer solution for at least 30 seconds. Do not use equipment/utensils not properly sanitized. Three bay is not set up. All three bays ate observed empty. Observed employee wash, rinse and run sanitizer over a french fry/cooked nugget scoop using the soapy hose the faucet and the sanitizer hose. Educated the employee that the three bays needed to be set up for warewashing. Bays set up to clean during inspection started. 400ppm quat. **Warning**
35A-02-5    High Priority - Live, small flying insects in kitchen, food preparation area, or food storage area. Approximately 5 flies between the ice machine and the dry storage rack in the back room over a puddle of standing water. **Warning**
03B-01-4    High Priority - Potentially hazardous (time/temperature control for safety) hot held at less than 135 degrees Fahrenheit or above. 101°F gravy, 105°F mashed potatoes 11:35am Items moved hot box. Second check gravy 115°F at 12:32. Operator to discard. **Repeat Violation** **Warning**
35A-23-4    High Priority - Roach excrement and/or droppings present. Approximately 6 dried on the back of the ice machine in the back room below the orange warning sign. **Warning**
01B-02-4    High Priority - Stop Sale issued on potentially hazardous (time/temperature control for safety) food due to temperature abuse. 101°F gravy, 105°F mashed potatoes 11:35am Items moved hot box. Second check gravy 115°F at 12:32 **Repeat Violation** **Warning**
29-34-4    High Priority - Vacuum breaker missing at hose bibb or on fitting/splitter added to hose bibb. Outside of building near dumpster. **Warning**
22-02-4    Intermediate - Accumulation of food debris/grease on food-contact surface. Multiple areas. On box cutter stored on food box storage rack outside the walk in cooler. On hot water unit and spigot next to the baine marie. Melted butter warming unit. Ice scoop and holder at the front line. Debris dried on front line hot table. Crumbs, debris. Bins for condiments at the drive thru with buildup of debris. Speed rack in the walk in unit. **Warning**
32-11-5    Intermediate - Lack of toilet tissue at each toilet. Soiled roll in women's restroom. **Warning**
53A-01-5    Intermediate - Manager lacking proof of food manager certification. A list of accredited food manager certification examination providers can be found at http://www.myfloridalicense.com/dbpr/hr/food-lodging/managercertification.html. **Warning**
31B-02-4    Intermediate - No paper towels or mechanical hand drying device provided at sole handwash sink in the kitchen and in the men's restroom. **Warning**
16-34-4    Intermediate - No quaternary ammonium chemical test kit provided when using quaternary ammonium chemical sanitizer at three-compartment sink/warewashing machine. **Warning**
