A summary of the violations found during the inspection are listed below.
The department cites violations of Florida's sanitation and safety laws,
which are based on the standards of U.S. Food and Drug Administration's Food Code.
High Priority violations are those which could contribute directly to a foodborne
illness or injury and include items such as cooking, reheating, cooling and hand-washing.
Intermediate violations are those which, if not addressed, could lead to risk factors
that contribute to foodborne illness or injury. These violations include personnel training,
documentation or record keeping and labeling. Basic violations are those which are considered
best practices to implement. While most establishments correct all violations in a timely
manner (often during the inspection), the division's procedures are designed to compel
compliance with all violations through follow-up visits, administration action or closure
when necessary.
Basic - Access panel in dry goods store room made of broken Sheetrock, bulging wall panel in dry goods storage room, bulging wall panel next to three compartment sink, bulging wall panel at entry to kitchen, hole in wall above electrical panel, holes in walls of electrical room, hole in floor on East end of cook line. **Warning**
Basic - Dead roaches on premises. Observed: one dead roach on top of plates on cook line, approximately three dead roaches beneath empty storage drawers on cook line, one dead roach in hand wash sink on East end of cook line. **Repeat Violation** **Admin Complaint**
High Priority - Basic - Food placed in soiled container/equipment. Cheeses stored on wire racks inside pans with water containing a mold like substance. **Warning**
High Priority - Roach activity present as evidenced by live roaches found. Observed one live roach at steam table on West end of cook line, three live roaches in empty storage drawers on cook line. **Repeat Violation** **Admin Complaint**
High Priority - Rodent activity present as evidenced by rodent droppings found. Dry droppings observed throughout kitchen and storage areas. Observed approximately: 25 dry rodent droppings on top of dish machine, 30 dry rodent droppings beneath drain boards of dish machine, 15 dry rodent droppings around baseboard of dry goods storage room, one dry rodent dropping on top of container of sugar in dry goods storage room, dry rodent droppings too numerous to count behind cook line equipment, 20 dry rodent droppings along baseboard of rear prep area of kitchen, 5 dry rodent droppings at hole in wall at entrance to kitchen, dry rodent droppings too numerous to count in electrical room on east end of kitchen. **Admin Complaint**
High Priority - Rodent burrow or rodent nesting materials present. Observed excessive insulation protruding from hole in bulging wall at entry to kitchen with rodent rub marks and dry rodent droppings. **Admin Complaint**
High Priority - Rodent rub marks present: along cove molding of kitchen, at hole in bulging wall panel at entry to kitchen, at plumbing hole beneath rinse sink at dish machine, at hole above electrical panel, continuing along conduit at ceiling, continuing down corner next to walk in cooler 4, at entry to hole in floor on East end of cook line. **Admin Complaint**
