Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

13-03-4    Basic - Employee with no hair restraint while engaging in food preparation. Cook no hair restraint.
14-11-4    Basic - Equipment in poor repair. Reach in deli style cooler 45°F.
36-18-4    Basic - Floor tiles cracked, broken or in disrepair. In kitchen area.
10-20-4    Basic - In-use tongs stored on equipment door handle between uses. Tongs on equipment door handle. Moved tongs to dish pit. **Corrective Action Taken**
51-13-4    Basic - No Heimlich maneuver/choking sign posted.
51-18-6    Basic - No copy of latest inspection report available.
33-16-4    Basic - Open dumpster lid.
42-01-4    Basic - Wet mop not stored in a manner to allow the mop to dry. Mop not hung to dry. **Repeat Violation**
01B-07-4    High Priority - Food with mold-like growth. See stop sale. Moldy tomato in reach in deli style cooler. Operator discarded.
35A-05-4    High Priority - Roach activity present as evidenced by live roaches found. 4 live roaches behind electrical box on wall behind reach in cooler in kitchen. 10 live roaches under and behind the 2 door reach in cooler in dining area near walkway to restrooms.
35A-23-4    High Priority - Roach excrement and/or droppings present. Roach excrement and droppings along pipes for electric in kitchen.
41-28-4    High Priority - Sanitizer not used in accordance with the manufacturer's recommendations. Chlorine Sanitizer at 0 ppm in 3 compartment sink. Corrected to 50ppm chlorine.
29-42-4    High Priority - Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet. Mop sink in rear near 3 compartment sink no vacuum breaker. **Repeat Violation**
31A-03-4    Intermediate - Handwash sink not accessible for employee use due to items stored in the sink. Pot in hand sink in kitchen.
16-35-4    Intermediate - No chlorine chemical test kit provided when using chlorine sanitizer at three-compartment sink/warewashing machine.
02C-04-4    Intermediate - Operator is not properly tracking/marking the number of days ready-to-eat, potentially hazardous (time/temperature control for safety) food prepared onsite was held at refrigeration temperatures prior to freezing in order to properly date mark the food when it is thawed and held at refrigeration temperatures again. Potatoes and garlic butter made onsite in cooler no date mark.
31B-05-4    Intermediate - Paper towel dispenser at handwash sink not working/unable to dispense paper towels. Unable to dispense paper towels in kitchen hand sink. **Repeat Violation**
03D-20-4    Intermediate - Potentially hazardous (time/temperature control for safety) food cooled in quantity deeper than 4 inches. Garlic butter in deep pan cooling. Educated on larger pans to cool more surface to cool. Moved to reach in freezer to quick chill.
03D-16-4    Intermediate - Potentially hazardous (time/temperature control for safety) food covered while cooling. Butter cooling covered. Operator uncovered and moved to reach in freezer for quick chill. **Corrective Action Taken**
41-17-4    Intermediate - Spray bottle containing toxic substance not labeled. Sprayer with pesticide in kitchen under 3 compartment sink no label.
