Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

36-24-5    Basic - Hole in or other damage to wall. Hole in wall in men's restroom near sit down stall.
22-19-4    Basic - Interior of microwave soiled with encrusted food debris. Interior of microwave in cooler area of kitchen soiled.
14-67-4    Basic - Reach-in cooler gasket torn/in disrepair. Torn gaskets on reach in deli style cooler, beverage air cooler. **Repeat Violation**
14-33-4    Basic - Reach-in cooler shelves with rust that has pitted the surface. Rust pitted reach in cooler racks on the beverage air cooler in kitchen near microwave.
35B-13-4    Basic - Screening is not 16-mesh to the inch. Screen in rear ice machine and reach in freezer storage area.
25-06-4    Basic - Single-service articles not stored inverted or protected from contamination. Plates on steam table not inverted. **Repeat Violation**
23-09-4    Basic - Soiled reach-in cooler gaskets. Multiple Reach in coolers gaskets black moldlike debris.

23-10-4    Basic - Walk-in cooler and/or walk-in freezer gaskets soiled with slimy/mold-like build-up. Moldlike debris on gaskets of walk in cooler.,
14-41-4    Basic - Walk-in cooler gasket torn/in disrepair. Walk in cooler gasket torn.
01B-01-4    High Priority - Dented/rusted cans present. See stop sale. Dented cans .
03A-02-4    High Priority - Potentially hazardous (time/temperature control for safety) food cold held at greater than 41 degrees Fahrenheit. Sliced cheese 45°F . Employee moved to reach in freezer for quick chill. Retemped cheese to 38°F. **Corrective Action Taken**
08A-18-5    High Priority - Raw animal foods not properly separated from one another based upon minimum required cooking temperature in reach-in freezer - all products not commercially packaged. Open chicken tenders over open onion rings and lasagne.
35A-04-4    High Priority - Rodent activity present as evidenced by rodent droppings found. 35 rodent droppings on dish machine 2 of which are soft rodent droppings, 33 are brown to dark brown and hard. 11 rodent droppings on top of sanitizer 4 semi soft , 7 hard and dry. 19 hard droppings on top of soap bucket. 12 rodent droppings 4 are semi soft on top of chemical drying agent in dish pit next to cookline near the hand sink the remaining 8 are hard and dry. 9 hard and dry rodent droppings on top of microwave oven in use. 7 droppings behind freezer in ice machine area cannot get to them to determine if soft but color is brown. 3 dark brown rodent droppings and hair on glue trap behind freezer in ice machine rear area . 1 dark brown hard rodent dropping in women's restroom next to sink.
12A-09-4    High Priority - Single-use gloves not changed as needed after changing tasks or when damaged or soiled. Employee went to walk in cooler then back to work no glove change or hand wash. Educated employee on glove changing and hand washing. **Corrective Action Taken**
22-26-4    Intermediate - Buildup of soiled material on racks in the walk-in cooler.
22-22-4    Intermediate - Encrusted material on can opener blade. Food debris buildup on can opener blade. **Repeat Violation**
31B-02-4    Intermediate - No paper towels or mechanical hand drying device provided at handwash sink. No paper towels at hand sink on cook line. Employee refilled paper towels. **Corrective Action Taken**
02C-04-4    Intermediate - Operator is not properly tracking/marking the number of days ready-to-eat, potentially hazardous (time/temperature control for safety) food prepared onsite was held at refrigeration temperatures prior to freezing in order to properly date mark the food when it is thawed and held at refrigeration temperatures again. Coleslaw in walk in cooler no date mark made yesterday. Am. **Repeat Violation**
53B-13-5    Intermediate - Proof of required state approved employee training not available for some employees. To order approved program food safety material, call DBPR contracted provider: Florida Restaurant and Lodging Association (SafeStaff) 866-372-7233. Employee for 7 years can't find training certificate and employee does not have card on her.
