Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

On concrete block under the three bay sink. 2 on the floor near the entrance of kitchen from front line. 4 on glue trap behind chest freezer under window exhaust fan unit. 1 behind upright beef freezer. 1 dead on powdered lemonade package on dry storage shelf. 3 dead in the corner near the mop sink. 1 dead in the mop sink drain. 4 dead roaches in the women's restroom, plus two alive and kicking but on their backs. 4 dead on the men's restroom floor, one alive on its back on the men's handsink. **Warning**
14-11-4    Basic - Equipment in poor repair. Door to reach in cooler by back door falls off bracket. **Warning**

36-22-4    Basic - Floor area(s) covered with standing water. Under 3 bay sink. Employee states drain pipe leaks under the right bay. **Warning**
36-73-4    Basic - Floor soiled/has accumulation of debris under two dry storage shelves in the back of the kitchen. **Warning**
23-22-4    Basic - Food debris/dust/soil residue on dry storage shelves above cookline prep table. **Warning**
36-14-4    Basic - Grease accumulated on kitchen floor and/or under cooking equipment at the fryer and stove. **Warning**
36-31-4    Basic - Hole in ceiling. In kitchen, attic access door is not sealed at the ceiling. Also, near chest freezers where wiring line runs thru. **Warning**
36-24-5    Basic - Hole in or other damage to wall where cookline pipe runs into wall on the side of the fryer. Also behind upright beef freezer above the outlet box. **Warning**

23-05-4    Basic - Soil residue build-up on pipes behind cooking equipment, under three bay and hand sink, and along kitchen walls. **Warning**
33-34-4    Basic - Storage area not maintained clean and organized. Wood for smoker not organized in back lawn. Pest droppings observed. Excessive amount of items in back lawn. Several items outside back door. **Warning**
22-08-4    Basic - Thick black sludge on interior smoker near grates and cooking meats. **Warning**
32-23-4    Basic - Toilet/urinal not flushing/functioning properly. Women's restroom. Nearly overflowed at flush attempt. Non-disposable product wrapper came up. **Warning**
36-02-5    Basic - Unsealed concrete floor in food preparation, food storage and warewashing areas. **Warning**
16-20-4    High Priority - Faucet at three-compartment sink does not reach all compartments. Only reaches the center bay. **Warning**
29-04-4    High Priority - Nonfood-grade hose conveying potable water. At spigot next to stove, used for the smoker grill. **Repeat Violation** **Warning**
03B-01-4    High Priority - Potentially hazardous (time/temperature control for safety) hot held at less than 135 degrees Fahrenheit or above. 107°F chicken fully cooked on top grill **Warning**
35A-05-4    High Priority - Roach activity present as evidenced by live roaches found. 1 observed walking on floor between smoker and chest freezers. 3 caught on glue trap behind chest freezer but still moving. 1 observed walking near the chain saw (employee stepped on this one to kill it.). One live roach on ceiling above dry storage. **Repeat Violation** **Warning**
35A-23-4    High Priority - Roach excrement and/or droppings present. Approximately 15 dried on shelf below the microwave. **Warning**
35A-04-4    High Priority - Rodent activity present as evidenced by rodent droppings found. 10 soft to semi-soft droppings on the upper hole of the concrete block under the three bay sink. Heavy droppings in flattop grease drain hole to collection tray below. Stove hot and in use. Too many to count on the stove grease drip catches under the active burners. Too many to count or reach for pinch test in hole to lower chest freezer. Approximately 40 dried droppings in the kitchen corner near the chain saw stored on the floor. Approximately 8 dried droppings on the pass thru window from kitchen to front line. **Warning**
35A-21-4    High Priority - Rodent burrow or rodent nesting materials present. Front of stove at pull-out drip pans is insulation/cotton type material in pieces. **Warning**
01B-02-4    High Priority - Stop Sale issued on potentially hazardous (time/temperature control for safety) food due to temperature abuse. 107°F chicken fully cooked on top grill **Warning**
29-34-4    High Priority - Vacuum breaker missing at hose bibb or on fitting/splitter added to hose bibb. At hose next to stove used to spray smoker. **Repeat Violation** **Warning**
