which are based on the standards of U.S. Food and Drug Administration's Food Code.
High Priority violations are those which could contribute directly to a foodborne
illness or injury and include items such as cooking, reheating, cooling and hand-washing.
Intermediate violations are those which, if not addressed, could lead to risk factors
that contribute to foodborne illness or injury. These violations include personnel training,
documentation or record keeping and labeling. Basic violations are those which are considered
best practices to implement. While most establishments correct all violations in a timely
manner (often during the inspection), the division's procedures are designed to compel
compliance with all violations through follow-up visits, administration action or closure
when necessary.

Must submit plans, plan review application and payment for plan review to DBPR H and R Plan Review office located at 2601 Blair Stone Rd., Tallahassee, FL 32399-1011. A wall was removed to add extra space for a warewashing area. The three compartment sink was removed from its original location, it's now adjacent to the back door of the kitchen. Another wall was removed to add a space for a dry storage room. A plan review application with plans of the remodeled kitchen must be submitted and approved in 30 days. - From follow-up inspection on 2017-07-05: Spoke with Plan Review and there was not any application or plans submitted and approved at this time. **Admin Complaint**

To order approved program food safety material, call DBPR contracted provider: Florida Restaurant and Lodging Association (SafeStaff) 866-372-7233. - From follow-up inspection on 2017-07-05: The Safe Staff certificate for Ehab Zakhary issue date 2/1/2014 and expiration 1/31/17.
