A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

51-09-4    Basic - - From inspection on 2017-05-02: Basic - Establishment did not report seating change that affects the license fee, Clean Indoor Air Act, sewage system approval or other related requirements. - From follow-up inspection on 2017-06-08: Observed seating change form signed already by the fire inspector, just waiting for sewer inspector to complete it. **Time Extended** - From follow-up inspection on 2017-07-17: According operator the seating change evaluation was submitted.
51-16-5    Intermediate - - From inspection on 2017-05-02: Intermediate - No plan review submitted and approved - renovations were made or are in progress. Must submit plans, plan review application and payment for plan review to DBPR H and R Plan Review office located at 2601 Blair Stone Rd., Tallahassee, FL 32399-1011. Observed the operator added a prep area and moved the ice machine to an unapproved room without a Handwash sink and submitting plans. - From follow-up inspection on 2017-06-08: **Time Extended** - From follow-up inspection on 2017-07-17: The operator removed the cooking equipment from the dry storage. Also the operator disconnected the ice machine even when they added a Handwash sink until get the approval from Tallahassee office.
