A summary of the violations found during the inspection are listed below.
The department cites violations of Florida's sanitation and safety laws,
which are based on the standards of U.S. Food and Drug Administration's Food Code.
High Priority violations are those which could contribute directly to a foodborne
illness or injury and include items such as cooking, reheating, cooling and hand-washing.
Intermediate violations are those which, if not addressed, could lead to risk factors
that contribute to foodborne illness or injury. These violations include personnel training,
documentation or record keeping and labeling. Basic violations are those which are considered
best practices to implement. While most establishments correct all violations in a timely
manner (often during the inspection), the division's procedures are designed to compel
compliance with all violations through follow-up visits, administration action or closure
when necessary.

Approximately eighteen dead roaches on floor in three compartment area.
Approximately five dead roaches on floor under storage shelf.

Basic - Employee beverage container in a food preparation/clean dishware area does not have a lid and a straw or equivalent dispensing mechanism to prevent hand contact with lip contact area of drink container.

High Priority - Roach activity present as evidenced by live roaches found.

Approximately thirteen live roaches on floor under reach on freezer and shelf.
Two live roaches on floor in front of walk in cooler entrance.
Approximately three live roaches behind cove molding and wall next to ice cream machine and ice machine.
Approximately six live roaches on floor under shelf next to ice machine.
Two live roaches on floor under soda boxes.
Approximately four live roaches on floor under dry storage shelf in front of ice machine.
At the drive thru window, approximately five live roaches on floor under table.
Approximately four live roaches on floor under hot holding boxes.
Approximately nine live roaches on floor under bread racks.
One live roach on mop sink.
