Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

33-11-4    Basic - Missing drain plug at dumpster.
36-66-4    Basic - Ventilation inadequate as evidenced by smoke in kitchen area.
12A-16-4    High Priority - Employee began working with food, handling clean equipment or utensils, or touching unwrapped single-service items without first washing hands. Employee scooping ice without washing hands first.
35A-02-5    High Priority - Live, small flying insects in kitchen, food preparation area, or food storage area. Back storage area.
08A-18-5    High Priority - Raw animal foods not properly separated from one another based upon minimum required cooking temperature in reach-in freezer - all products not commercially packaged. Observed open bag of raw nuggets over open bag of RTE French fries.
01B-03-4    High Priority - Stop Sale issued due to adulteration of food product. Bag of shredded cheese contaminated by water dripping on it from the walk in cooler fans.
29-34-4    High Priority - Vacuum breaker missing at hose bibb or on fitting/splitter added to hose bibb. In mop sink area.
22-24-4    Intermediate - Accumulation of black/green mold-like substance on/around soda dispensing nozzles. Front counter. **Corrected On-Site**
11-07-4    Intermediate - Certified Food Manager or person in charge lacks knowledge of foodborne illnesses and symptoms of illness that would prevent an employee from working with food, clean equipment and utensils, and single-service items.
31A-09-4    Intermediate - Handwash sink not accessible for employee use at all times. Prep table and other equipment blocking access.
27-16-4    Intermediate - Hot water not provided/shut off at employee handwash sink. Neither male or female bathrooms used by employees have hot water.
29-28-4    Intermediate - Water treatment device has not been inspected or serviced according to manufacturer's instructions. Observed noted on water filter was last replaced in 03/2011.
