Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

35B-01-4    Basic - Exterior door has a gap at the threshold that opens to the outside.
36-18-4    Basic - Floor tiles cracked, broken or in disrepair. Left of flat top grill. Under sink at end of line. Tile in dish area. **Repeat Violation**
08B-30-4    Basic - Food stored in dry storage area not covered. Sugar like substance in square container covered with round lid. Not completed covered or protected. Foreign material in container.
33-19-4    Basic - Garbage on the ground and/or pad around dumpster.
36-31-4    Basic - Hole in ceiling. Above walkin door.
36-24-5    Basic - Hole in or other damage to wall. Tile falling off wall under white shelves in storage area.
14-42-4    Basic - Hood filter missing from automatic fire suppression/exhaust system. Hood filters missing baffles. 5 units missing baffles. Space between filters in small hood.
33-33-4    Basic - Large amount of unused equipment/supplies present. Back and side of restaurant.
38-12-4    Basic - Light shield for lights in food preparation, food storage or warewashing area missing end caps. Line area.
29-49-5    Basic - Observed standing water in bottom of reach-in cooler. Cooler left of flat top grill.
33-16-4    Basic - Open dumpster lid. **Corrected On-Site**
14-67-4    Basic - Reach-in cooler gasket torn/in disrepair. Cooler left of grill. Gasket for chilled glasses torn. **Repeat Violation**
25-05-4    Basic - Single-service articles improperly stored. Soufflé cups on floor in store room.
29-03-4    Basic - Water draining onto floor surface. Hand sink outside walk in cooler.
02D-01-5    Basic - Working containers of food removed from original container not identified by common name. Sugar like substance in dry room.
22-41-4    High Priority - Dishmachine chlorine sanitizer not at proper minimum strength. Discontinue use of dishmachine for sanitizing and set up manual sanitization until dishmachine is repaired and sanitizing properly. Chlorine is feeding but won't register 50ppm.
03A-02-4    High Priority - Potentially hazardous (time/temperature control for safety) food cold held at greater than 41 degrees Fahrenheit. Salsa for chips at 60 °F , employee put in cooler. **Corrective Action Taken**
35A-09-4    High Priority - Presence of insects, rodents, or other pests. Live Small black insects emerging from corn mesa flour and all purpose flour. Approximately 40 .
35A-05-4    High Priority - Roach activity present as evidenced by live roaches found. 8 live cockroaches in three door cooler not functioning across from cooks line. Found in gaskets.
35A-04-4    High Priority - Rodent activity present as evidenced by rodent droppings found. 12 dry rodent dropping in storage area on white shelf.
01B-03-4    High Priority - Stop Sale issued due to adulteration of food product. Corn mesa flour and all purpose flour have insects crawling out of bags. Insects have made holes in bags. Operator discarded. **Corrective Action Taken**
22-21-4    Intermediate - Accumulation of black/green mold-like substance inside the ice bin.
22-02-4    Intermediate - Accumulation of food debris/grease on food-contact surface. Baskets used for chips have food debris in most containers. Approximately 20 baskets.
53A-05-5    Intermediate - No currently certified food service manager on duty with four or more employees engaged in food preparation/handling. 31B-02-4    Intermediate - No paper towels or mechanical hand drying device provided at handwash sink. **Corrected On-Site** **Repeat Violation**
53B-13-5    Intermediate - Proof of required state approved employee training not available for some employees. To order approved program food safety material, call DBPR contracted provider: Florida Restaurant and Lodging Association (SafeStaff) 866-372-7233.
02C-02-4    Intermediate - Ready-to-eat, potentially hazardous (time/temperature control for safety) food prepared onsite and held more than 24 hours not properly date marked. Sauce buckets in cooler have no date markings on them.
41-17-4    Intermediate - Spray bottle containing toxic substance not labeled. Purple liquid in bottle in dish area not labeled.
