Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

36-24-5    Basic - Hole in or other damage to wall over 3 bay sink. **Warning**
22-19-4    Basic - Interior of microwave soiled with encrusted food debris. **Warning**
38-07-4    Basic - Lights in food preparation, food storage or warewashing area missing the proper shield, sleeve coatings or covers. Over cookline. **Warning**
14-31-4    Basic - Nonfood-grade bags used in direct contact with food. Black plastic bags covering bread. **Warning**
25-06-4    Basic - Single-service articles not stored inverted or protected from contamination. Take out containers on bulk bin in open bag, not inverted. **Warning**
23-05-4    Basic - Soil residue build-up on interior of oven that is being used to hold bags of dry pasta. **Warning**
36-10-4    Basic - Soiled floor under ware wash sink and dish machine. Heavy buildup. **Warning**
36-30-4    Basic - Wall not smooth, nonabsorbent and easily cleanable in food preparation, food storage, warewashing areas or bathroom. Behind cookline cooler, wood wall exposed. **Warning**
36-27-5    Basic - Wall soiled with accumulated grease, food debris, and/or dust. Behind slicer. **Warning**
29-11-4    Basic - Water leaking from pipe and/or faucet/handle below the three bay sink. Sink is dripping into a pan full of dirty water. **Warning**
02D-01-5    Basic - Working containers of food removed from original container not identified by common name. Salt in pitcher and white product in Bulk bins. **Warning**
01B-01-4    High Priority - Dented/rusted cans present. See stop sale. 1-#10 can peeled tomatoes dented at the seam. **Warning**
12A-18-4    High Priority - Employee dried hands on clothes/apron/soiled towel after washing. No paper towel present at only kitchen hand sink. Observed cook and server wash hands and dry hands on apron/cloth towels. Operator put a stack of napkins nearby but other employees are not using them. **Warning**
12A-07-4    High Priority - Employee failed to wash hands before changing gloves and/or putting on gloves to work with food. **Warning**
09-01-4    High Priority - Employee touching ready-to-eat food with their bare hands - food was not being heated as a sole ingredient to 145 degrees F or immediately added to other ingredients to be cooked/heated to the minimum required temperature to allow bare hand contact. Establishment has no approved Alternative Operating Procedure. Operator checked coldness of cooked pasta with bare hands and is cooking new pasta because pasta is frozen in the make table and not up to his specifications. **Warning**
35A-09-4    High Priority - Presence of insects, rodents, or other pests. 2 small flies at kitchen handwash sink between cooler units. Ants observed on the dessert cooler at the front line. 1 fly in the men's restroom. **Warning**
01B-14-4    High Priority - Shell eggs in use or stored with cracks or broken shells. See stop sale. 2 eggs **Warning**
01B-02-4    High Priority - Stop Sale issued on potentially hazardous (time/temperature control for safety) food due to temperature abuse. 67°F fresh garlic in olive oil. Educated staff who immediately discarded the garlic and oil and made a plan to change the process and cold hold in the table. **Warning**
01C-03-4    Intermediate - Clam/mussel/oyster tags not marked with last date served. Educated operator. **Corrective Action Taken** **Warning**
22-22-4    Intermediate - Encrusted material on can opener blade. **Warning**
31A-07-4    Intermediate - Handwash sink missing in warewashing or food preparation area. Front line where salad prep cooler and ice scooping take place. **Warning**
31B-02-4    Intermediate - No paper towels or mechanical hand drying device provided at handwash sink. Kitchen handwash sink between coolers. **Warning**
51-16-5    Intermediate - No plan review submitted and approved - renovations were made or are in progress. Must submit plans, plan review application and payment for plan review to DBPR H and R Plan Review office located at 2601 Blair Stone Rd., Tallahassee, FL 32399-1011. Establishment layout does not match what the current layout looks like. A floor mixer is in the what the kitchen bathroom area by the back door used to be. A salad prep cooler is up front with a door between the kitchen and the front. **Warning**

22-07-4    Intermediate - Slicer blade and guard soiled with old food debris. Clean plates observed stored on the slicer. It is not is use for food prep. **Warning**
22-25-4    Intermediate - Soiled, rusty utensil used to clean the stove top is observed stored in the clean, mounted mixer bowl. The tool was removed but the bowl needs cleaning and sanitizing. **Warning**
