Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

35A-03-4    Basic - Dead roaches on premises. Observed approximately five dead roaches next to slicer prep table in kitchen. Observed approximately 5 dead roaches in a controlled device next to prep table in main kitchen. observed approximately 7 dead roaches in prep room behind ware washing area. Observed one dead roach under shelving over three compartment sink in warewashing area.
13-03-4    Basic - Employee with no hair restraint while engaging in food preparation. Observed employee in coffee station with no hair restraint.
35B-01-4    Basic - Exterior door has a gap at the threshold that opens to the outside. Observed gaps and holes in side exterior door to back lot.

10-21-4    Basic - Ice scoop stored on top of dirty ice machine between uses. Ice machine stored outside exposed to splash.

29-08-4    Basic - Plumbing system in disrepair. Observed handwashing sink in front bar leaking water.
14-41-4    Basic - Walk-in cooler gasket torn/in disrepair in reach in cooler in kitchen across from stove.
21-12-4    Basic - Wet wiping cloth not stored in sanitizing solution between uses. Observed wet wiping cloth on front counter area on top of food prep area and in kitchen prep area. **Repeat Violation** **Admin Complaint**
14-06-4    Basic - Wood food-contact surface not properly sealed. Wood Rice bin in back storage area not sealed.
10-05-4    High Priority - In-use utensil stored in unclean water at or above 135 degrees Fahrenheit.
35A-02-5    High Priority - Live, small flying insects in kitchen, food preparation area, or food storage area. Observed approximately 3 lives flies in front Coffee station and kitchen area.
03A-02-4    High Priority - Potentially hazardous (time/temperature control for safety) food cold held at greater than 41 degrees Fahrenheit. Observed cooked pork in reach in cooler in coffee station at 48°f, less than 1 hour.
03B-01-4    High Priority - Potentially hazardous (time/temperature control for safety) hot held at less than 135 degrees Fahrenheit or above. Observed croquettes in front coffee station reach in warmer at 125°, less than 1 hour.
08A-05-4    High Priority - Raw animal food stored over ready-to-eat food. Observed raw pork stored over ready to eat rice and beans and cook chicken inside walk in cooler.
08A-20-4    High Priority - Raw animal foods not properly separated from each other in holding unit. Observed raw fish stored with raw pork inside walk in cooler.
35A-05-4    High Priority - Roach activity present as evidenced by live roaches found. Observed approximately one live roach in hinge on door of reach in cooler across from stove in kitchen.
22-02-4    Intermediate - Accumulation of food debris/grease on food-contact surface. Observed soiled bread oven in kitchen. Observed soiled prep table in coffee station. Observed ice cream melted inside of reach in freezer in coffee station. Slicer blade soiled with old food debris.Accumulation of black/green mold-like substance in the interior of the ice machine.
11-07-4    Intermediate - Certified Food Manager or person in charge lacks knowledge of foodborne illnesses and symptoms of illness that would prevent an employee from working with food, clean equipment and utensils, and single-service items. Discussed the big 5 food borne illness with person in charge. Provided educational flyer. **Corrective Action Taken**
31A-03-4    Intermediate - Handwash sink not accessible for employee use due to items stored in the sink in back bar area. **Repeat Violation**
31A-04-4    Intermediate - Handwash sink removed from food preparation/dishwashing area. Must be reinstalled in the same location where removed. Observed hand wash sink removed next to three compartment sink.
51-16-5    Intermediate - No plan review submitted and approved - renovations were made or are in progress. Must submit plans, plan review application and payment for plan review to DBPR H and R Plan Review office located at 2601 Blair Stone Rd., Tallahassee, FL 32399-1011. Observed new food preparation area with hot holding equipment and cold holding equipment in side of dining area. Area was created as a small coffee/sandwich area. Observed a hand sink, a chest freezer for ice cream, hot holding units with food stored inside; a display case for produce and a toaster and coffee machine. **Repeat Violation**
53B-02-5    Intermediate - No proof of required state approved employee training provided for employee hired more than 60 days ago. To order approved program food safety material, call DBPR contracted provider: Florida Restaurant and Lodging Association (SafeStaff) 866-372-7233.. As per employee, Yanelis, she has been working in establishment over 60 days. According to employee, she has been at the location for over a year. Ar the time of inspection , she had no proof of employee training. **Repeat Violation** **Admin Complaint**
31B-03-4    Intermediate - No soap provided at handwash sink in coffee station.
02C-02-4    Intermediate - Ready-to-eat, potentially hazardous (time/temperature control for safety) food prepared onsite and held more than 24 hours not properly date marked. Observed in walk in cooler cooked pork, shredded beef and cheese not date marked prepared more than 24 hours as per employee.
