Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

35A-03-4    Basic - Dead roaches on premises. 3 dead roaches in glue trap in dry storage. 2 dead roaches in hall near walk in freezer, 9 dead roaches in electrical room near walk in freezer,
12B-07-4    Basic - Employee beverage container on a food preparation table or over/next to clean equipment/utensils. Employee drink on clean dish table in kitchen cook area. Employee moved drink to designated area. **Corrective Action Taken** **Repeat Violation**

36-73-4    Basic - Floor soiled/has accumulation of debris. Floor soiled near soda fountain in wait station.
36-31-4    Basic - Hole in ceiling. Ceiling hole in electrical room near walk in freezer. Ceiling hole between cook line and dish area foam is broken and glue traps below hole. Hole in ceiling over clean dish rack in hall to dry storage area.
10-20-4    Basic - In-use tongs stored on equipment door handle between uses. Tongs on oven door.
14-67-4    Basic - Reach-in cooler gasket torn/in disrepair. Reach in cooler gasket torn on reach in cooler on cook line. **Repeat Violation**
35B-13-4    Basic - Screening is not 16-mesh to the inch. Screen on top area of ice machine room not 16- mesh. **Repeat Violation**
23-08-4    Basic - Soda gun holster with accumulated slime/debris. Mold like debris on holster.
23-09-4    Basic - Soiled reach-in cooler gaskets. Soiled reach in cooler gaskets on cook line.
12A-12-4    High Priority - Employee switched from working with raw food to ready-to-eat food without washing hands. Employee touched raw fish to put in frier then took out fried fish with same gloved hand to plate. No hand wash or glove change.
01B-17-4    High Priority - Potentially hazardous (time/temperature control for safety) food required to be date marked is not date marked and time of opening/preparation cannot be determined. See stop sale. Spaghetti no date mark in walk in cooler.
03A-19-4    High Priority - Raw fish cold held at greater than 41 degrees Fahrenheit.1:00pm trout (46°F - Cold Holding); 1:00pm Grouper (45°F - Cold Holding) in reach in deli style cooler. operator moved fish to walk in freezer for quick chill. retemped at 1:30 both items at 40°F . **Corrective Action Taken** **Repeat Violation**
02C-01-4    High Priority - Ready-to-eat, potentially hazardous (time/temperature control for safety) food marked with a date that exceeds 7 days after opening/preparation. Couscous dated 7/15 in walk in cooler. Operator discarded. Pasta date marked 7/15 in walk in cooler. Operator discarded pasta.
01B-24-4    High Priority - Ready-to-eat, potentially hazardous (time/temperature control for safety) food not consumed/sold within 7 days after opening/preparation. See stop sale. Couscous not used in 7 days from made date 7/15.
03G-11-4    High Priority - Reduced oxygen packaged fish packaged onsite no longer frozen and not removed from reduced oxygen package. Tuna thawed in ROP packaging. Operator scored tuna packs. **Corrective Action Taken**
35A-05-4    High Priority - Roach activity present as evidenced by live roaches found.1 live roach in prep area near slicer. 1 live roach roach near walk in near walk in freezer.
35A-04-4    High Priority - Rodent activity present as evidenced by rodent droppings found.1 soft rodent dropping, 5 hard rodent droppings in dry storage area near cat fish food for exterior lake feeding by customers, 9 rodent droppings in electrical room near walk in freezer inspector unable to reach to determine if hard or soft, 1 soft rodent dropping behind table with hot holding steam table. **Repeat Violation**
01B-13-4    High Priority - Stop Sale issued due to food not being in a wholesome, sound condition. Couscous in reach in deli style reach in cooler had dead fly on top. Operator discarded couscous.
41-27-4    High Priority - Wiping cloth sanitizer solution exceeds the maximum concentration allowed. Chlorine 200ppm corrected to 75ppm.
01C-03-4    Intermediate - Clam/mussel/oyster tags not marked with last date served. Mussels tags no last used date.
31A-13-4    Intermediate - Employee used handwash sink as a dump sink. Hand wash sink used for dump sink in bar area.
22-22-4    Intermediate - Encrusted material on can opener blade. Can opener blade has food debris in prep area of kitchen.
53A-05-5    Intermediate - No currently certified food service manager on duty with four or more employees engaged in food preparation/handling. 02C-02-4    Intermediate - Ready-to-eat, potentially hazardous (time/temperature control for safety) food prepared onsite and held more than 24 hours not properly date marked. Spaghetti no date mark in walk in cooler. Made yesterday operator date marked. **Corrective Action Taken**
53B-05-5    Intermediate - Required employee training expired for all employees. To order approved program food safety material, call DBPR contracted provider: Florida Restaurant and Lodging Association (SafeStaff) 866-372-7233. Employees all expired.
