Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

21-12-4    Basic - Wet wiping cloth not stored in sanitizing solution between uses.
02D-01-5    Basic - Working containers of food removed from original container not identified by common name. Oil, sauce, salt container no labels.
50-08-6    High Priority - Establishment operating without a license from the Division of Hotels and Restaurants. Operator must obtain a public food service license from the Division of Hotels and Restaurants within 30 days. 03B-01-4    High Priority - Potentially hazardous (time/temperature control for safety) hot held at less than 135 degrees Fahrenheit or above. Cooked mushrooms 93°F at cook line next to grilled. Operator move the mushrooms to hot part of the grilled. **Corrective Action Taken**
41-09-4    High Priority - Toxic substance/chemical stored in food preparation area with potential to cause cross contamination. Observed insecticide on top of the slicer machine
22-26-4    Intermediate - Buildup of soiled material on racks in the reach-in cooler.
31A-13-4    Intermediate - Employee used handwash sink as a dump sink. Hand sink observed 2 cups and waste inside the hand sink

22-28-4    Intermediate - Interior of reach-in cooler soiled with accumulation of food residue. Also chest freezer at front prep area.
05-08-4    Intermediate - No probe thermometer provided to measure temperature of food products.
16-34-4    Intermediate - No quaternary ammonium chemical test kit provided when using quaternary ammonium chemical sanitizer at three-compartment sink/warewashing machine. **Repeat Violation**
