A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

36-34-5    Basic - Ceiling/ceiling tiles/vents soiled with accumulated food debris, grease, dust, or mold-like substance. Throughout kitchen and dining room.
12B-12-4    Basic - Employee beverage container in a food preparation/clean dishware area does not have a lid and a straw or equivalent dispensing mechanism to prevent hand contact with lip contact area of drink container. Can of soda on prep table. Discarded by operator. **Corrected On-Site**

35B-01-4    Basic - Exterior door has a gap at the threshold that opens to the outside. At back door.
36-41-4    Basic - Fan cover in walk-in cooler/freezer has accumulation of mold, dust and debris. Walk in cooler fans.
33-09-4    Basic - Garbage not placed in a receptacle for storage until pick up to make the garbage inaccessible to insects and rodents. Excessive garbage outside of back kitchen door. Operator began to throw garbage into dumpster. **Corrective Action Taken**
36-14-4    Basic - Grease accumulated on kitchen floor and/or under cooking equipment Throughout establishment especially in main kitchen area.
22-08-4    Basic - Interior of oven has heavy accumulation of black substance/grease/food debris. Oven on main cook line.
14-31-4    Basic - Nonfood-grade bags used in direct contact with food. Cooked rice stored in bag at bottom area of reach in cooler. **Repeat Violation**
21-11-4    Basic - Sanitizing solution for wiping cloths not free of food debris and visible soil. Bucket of wet wiping cloths in soiled solution water. Operator stated it was from the hood cleaning that took place the prior evening. Bucket discarded by operator. **Corrective Action Taken**
08B-13-4    Basic - Stored food not covered in walk-in cooler. Bus tub of lettuce/salad mix.
23-12-4    Basic - Walk-in cooler and/or walk-in freezer shelves soiled with encrusted food debris. Plastic shelf liners soiled in walk in cooler.
01B-07-4    High Priority - Food with mold-like growth. See stop sale. Bananas, ginger and cantaloupe in walk in cooler with mold like growth. Operator voluntarily discarded. **Corrective Action Taken**
08A-10-4    High Priority - Raw animal food stored over ready-to-eat food in reach-in cooler. Shelled eggs over ready to eat pre-set salad in glass front reach in cooler. Moved to bottom shelf by operator. **Corrective Action Taken**
22-21-4    Intermediate - Accumulation of black/green mold-like substance inside the ice bin. Interior of ice bin cover.
22-24-4    Intermediate - Accumulation of black/green mold-like substance on/around soda dispensing nozzles. Excessive build up on soda dispensing nozzles.
22-02-4    Intermediate - Accumulation of food debris/grease on food-contact surface. Dough mixer next to stove with significant grease accumulation inside.
31B-02-4    Intermediate - No paper towels or mechanical hand drying device provided at handwash sink. Hand wash sink nearest to walk in cooler.
31B-03-4    Intermediate - No soap provided at handwash sink. Hand wash sink nearest to walk in cooler.
