Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

38-01-4    Basic - Light shield damaged/in disrepair above cook line and in dish washing area.
14-31-4    Basic - Nonfood-grade bags used in direct contact with food. Take out bags used to store calamari in reach in freezer by cookline.
25-05-4    Basic - Single-service articles improperly stored. Take out boxes on floor in outdoor storage.
23-05-4    Basic - Soil residue build-up on nonfood-contact surface. Gaskets soiled at outside reach in freezer.
14-12-4    Basic - Utensils in poor condition. Chipped spatulas in dishwashing area.
02D-01-5    Basic - Working containers of food removed from original container not identified by common name. Container of sugar in prep area without label.
35A-02-5    High Priority - Live, small flying insects in kitchen, food preparation area, or food storage area. Observed approximately 115 live small flying insects by dishwashing/prep area, 16 by cook line, 9 by wait station , 15 in dining room and 28 by bar. Observed flying insects landing on clean utensils and equipment as well as on prep tables.

52-01-4    Intermediate - Identity of food or food product misrepresented. Menu indicates Caesar dressing is homemade. Observed commercial made Caesar dressing in walk in cooler. Cook states no other ingredients are added to dressing.
