Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

28-13-4    High Priority - Observed gray water outside back door area, at end of paved surface, where neighboring empty lot meets back walkway. Observed bubbling from underneath, and with odor. Employees do not need to cross this area to access hose at back entry and instructed employees to enter/ exit facility only from the front door. if they Flushed toilets, ran sinks and no backup observed inside establishment. No change in wastewater appearance/ amount when toilets flushed **Warning**
03A-02-4    High Priority - Potentially hazardous (time/temperature control for safety) food cold held at greater than 41 degrees Fahrenheit. Sliced tomatoes 43-49° over stocked in pan . Manager states they were pulled at 10 am. Recommend rapid chill Walk in cooler Mediterranean dip (46°F - Cold Holding); pulled pork (45°F - Cold Holding); cooked pasta (48°F - Cold Holding); veggie burger (44°F - Cold Holding); raw salmon (42°F - Cold Holding); Neptune crab meat (44°F - Cold Holding); ground camel (44°F - Cold Holding); creme brulee (44°F - Cold Holding); sliced tomatoes (45°F - Cold Holding); gravy (47°F - Cold Holding). Operator states they had door open to stock line at 10:00 am. Rechecked food after 1 hour observed dropping 1°F , repair man arrived for different cooler and cleaned walk in cooler coils, adjusted temperature. Rechecked after 30 minutes found items between 42°-46° pizza prep station salad cooler lift top onions (46°F - Cold Holding feta (46°F - Cold Holding); pizza make cooler pulled pork (49°F - Cold Holding); raw chorizo (46°F - Cold Holding); diced tomatoes (44°F - Cold Holding); Alfredo sauce (47°F - Cold Holding);. Operator states they arrived and turned on cooler at 10:00 am and began stocking product around 10:15. Recommend rapid chill. And do not stock cooler until ambient temperature at 41°F Cookline open top ; open cold bar sliced tomatoes (43-49°F - Cold Holding); cut lettuce (46-53°F - Cold Holding); Housemade remoulade (50°F - Cold Holding); sweet chili dressing (57°F - Cold Holding); garlic cilantro dressing Housemade (54°F - Cold Holding); cooler drawers pulled pork (48°F - Cold Holding); veggie patties (48°F - Cold Holding); raw ground beef (48°F - Cold Holding); raw chicken (58°F - Cold Holding); Alfredo sauce (45°F - Cold Holding); sweet chili sauce with fresh garlic (47°F - Cold Holding) sauces from overnight , drawers and open cooler stocked within the last hour. Manager arrived and provided proof that garlic in sauces is dried not fresh. Garlic infused oil, Housemade 83° from overnight **Corrective Action Taken** **Warning**
