Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

23-09-4    Basic - Soiled reach-in cooler gaskets. Observed soiled reach in cooler gaskets next to pizza oven. **Repeat Violation**
02D-01-5    Basic - Working containers of food removed from original container not identified by common name. Observed container of salt is not properly label.
03A-08-4    High Priority - Cooked meat/poultry cold held at greater than 41 degrees Fahrenheit. Observed cooked meatballs at 47°F in single door reach in cooler on cook line next to pizza oven. Employee moved meatballs to walk in cooler for rapidly cooling. **Corrective Action Taken**
35A-02-5    High Priority - One live small flying insects in wait station.
01B-02-4    High Priority - Stop Sale issued on potentially hazardous (time/temperature control for safety) food due to temperature abuse. Observed fresh garlic with oil mixed from 7.30.17 at 61°. Issue stop sale. Employee discarded. **Corrective Action Taken**
22-20-4    Intermediate - Accumulation of black/green mold-like substance in the interior of the ice machine. Observed black mold like substance inside of ice machine. Employee began the cleaning process. **Corrective Action Taken**
02C-03-4    Intermediate - Commercially processed ready-to-eat, potentially hazardous (time/temperature control for safety) food opened and held more than 24 hours not properly date marked after opening. Observed an opened gallon of whole milk not properly date marked in reach in cooler at wait station. Employee stated, it was opened yesterday 7.30.17.
22-22-4    Intermediate - Encrusted material on can opener blade. Observed can opener has encrusted material. **Repeat Violation**
31B-03-4    Intermediate - No soap provided at handwash sink. Observed no soap at front counter hand wash sink. Manager bought soap. **Corrected On-Site**
14-77-5    Intermediate - Reach-in cooler not maintained in good repair. Do not store potentially hazardous (time/temperature control for safety) food in this unit until the unit is repaired. Observed reach in cooler next to pizza oven has an ambient temperature of 55°F and standing water at the bottom of it. Establishment has three other reach in coolers and a walking cooler.
02C-02-4    Intermediate - Ready-to-eat, potentially hazardous (time/temperature control for safety) food prepared onsite and held more than 24 hours not properly date marked. Observed cooked pasta, lasagna stored in freezer not properly dare marked.
31A-02-4    Intermediate - Service area handwash sink not accessible for employee use due to being blocked by garbage can. **Repeat Violation**
