Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

31B-04-4    Basic - No handwashing sign provided at a hand sink used by food employees. Kitchen
35B-05-4    Basic - Outer openings not protected during operation and vermin and/or environmental cross contamination present. Fly fans not operational at time of inspection and front doors open.
29-11-4    Basic - Water leaking from pipe and/or faucet/handle. Spray hose at dish machine.
02D-01-5    Basic - Working containers of food removed from original container not identified by common name. Bulk bin next to brown sugar not labeled.
01B-01-4    High Priority - Dented/rusted cans present. See stop sale. Rusted and dented can of evaporated milk.
35A-02-5    High Priority - Live, small flying insects in kitchen, food preparation area, or food storage area. Approximately 10 small flies in tea area , 2 small flies in small room off kitchen near dirty linens, approximately 10+ in dish area on floor. Approximately 40+in bar area. 1 dead fly in ice bin at bar.
03A-02-4    High Priority - Potentially hazardous (time/temperature control for safety) food cold held at greater than 41 degrees Fahrenheit. Butter milk batter in ice bath at 58°f moved to working cooler. **Corrective Action Taken**
01B-24-4    High Priority - Ready-to-eat, potentially hazardous (time/temperature control for safety) food not consumed/sold within 7 days after opening/preparation. See stop sale. Cooked pasta dated 6/19/18/
01B-13-4    High Priority - Stop Sale issued due to food not being in a wholesome, sound condition. Dead fly in drink ice at bar area, 4 dead flies in bottle of grenadine in bar area.
22-20-4    Intermediate - Accumulation of black/green mold-like substance in the interior of the ice machine.
22-28-4    Intermediate - Interior of reach-in cooler soiled with accumulation of food residue. Non workin reach in cooler across from three compartment sink.

16-36-4    Intermediate - No color/concentration comparison chart available for chemical test kit used to check sanitizer. For chlorine test strips.
