Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

23-15-4    Basic - Accumulation of food debris/soil residue on handwash sink in the kitchen.
14-01-4    Basic - Bowl or other container with no handle used to dispense food. Observed bowls are being used as scoops for bit raw and cooked rice.
24-05-4    Basic - Clean glasses, cups, bowls, plates, pots and pans not stored inverted or in a protected manner. Clean dishes on the shelf next to the ice machine.
40-06-4    Basic - Employee personal items stored in or above a food preparation area. Observed a cell phone on a shelf on the cooks line. It was put away. **Corrected On-Site**
36-22-4    Basic - Floor area(s) covered with standing water on the cooks line.
10-20-4    Basic - In-use tongs stored on equipment door handle between uses. Observed multiple sets of tongs on oven door handles. They were removed. **Corrective Action Taken**
38-07-4    Basic - Lights in food preparation, food storage or warewashing area missing the proper shield, sleeve coatings or covers above the cooks line.
29-08-4    Basic - Plumbing system in disrepair. Observed water dripping from the walkin cooler condenser onto covered food.
08A-26-4    Basic - Raw animal food stored above unwashed produce. Raw Eggs above unwashed produce in the walkin cooler. The eggs were moved below the produce. **Corrected On-Site**
25-06-4    Basic - Single-service articles not stored inverted or protected from contamination. Observed take out containers on a cooler not stored inverted. They were turned over. **Corrected On-Site**
29-20-5    Basic - Standing water or very slow draining water in handwash sink in the kitchen.
08B-13-4    Basic - Stored food not covered in walk-in cooler. Cooked yucca and cooked potatoes. They were immediately covered. **Corrected On-Site**
36-26-4    Basic - Wall soiled with accumulated black debris in dishwashing area.
50-17-2    High Priority - High - Operating with an expired Division of Hotels and Restaurants license. License was paid during the inspection, confirmation 177047261 for $323. 00
22-41-4    High Priority - Dishmachine chlorine sanitizer not at proper minimum strength. Discontinue use of dishmachine for sanitizing and set up manual sanitization until dishmachine is repaired and sanitizing properly. Observed 0 ppm when tested. Chef primed the sanitizer and it retested at 50 ppm . **Corrected On-Site**
12A-07-4    High Priority - Employee failed to wash hands before changing gloves and/or putting on gloves to work with food. Observed cook wearing gloves handled soiled items and changed gloves without washing hands. He then removed the gloves, properly washed hands and put on new gloves. . If future hand wash violations are cited, an additional hand wash sink may be required to be installed . **Corrected On-Site**
10-05-4    High Priority - In-use utensil stored in unclean water below 135 degrees Fahrenheit. Observed 2 knives in a container of 84F water. The water was dumped and the knives sent to be cleaned. **Corrective Action Taken**
03A-02-4    High Priority - Potentially hazardous (time/temperature control for safety) food cold held at greater than 41 degrees Fahrenheit. Observed an overloaded pan of cooked chicken in the cooks line flip top cooler is43-51F. Also observed a doubled pan of butter is 51F. Some of the chicken and the butter were moved into the lower part of the flip top cooler. **Corrective Action Taken**
52-04-4    Intermediate - Establishment advertised mahi for 6 different dishes on the menu/menu board but served another type of fish. Establishment does not have any mahi on site or any invoices less than 30 days old with mahi on the invoice. Only observed raw grouper, raw Swai, raw tuna, raw snapper and raw salmon in the establishment. **Admin Complaint**
31B-02-4    Intermediate - No paper towels or mechanical hand drying device provided at handwash sink in the kitchen. Paper towels were placed by this sink. **Corrected On-Site**
16-34-4    Intermediate - No quaternary ammonium chemical test kit provided when using quaternary ammonium chemical sanitizer at three-compartment sink/warewashing machine.
