Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

40-01-4    Basic - No suitable facilities provided to store employee clothing and other possessions. Jacket hanging on soap dispenser in dish area. Backpack on top of calamari flour bin. **Warning**
14-40-4    Basic - Nonfood-contact equipment in poor repair. Reach in cooler missing handle. **Warning**
42-01-4    Basic - Wet mop not stored in a manner to allow the mop to dry. Mop head in mop sink. **Warning**
21-08-4    Basic - Wiping cloth quaternary ammonium compound sanitizing solution not at proper minimum strength. All buckets quat 0 ppm. Inspector tried both chlorine and quat strips. Dispenser is not functioning properly. Sanitizer container below three compartment sink gave reading of 400 ppm. **Warning**
21-38-4    Basic - Wiping cloth sanitizing solution stored on the floor. Near cook line in hallway. **Warning**
22-41-4    High Priority - Dishmachine chlorine sanitizer not at proper minimum strength. Discontinue use of dishmachine for sanitizing and set up manual sanitization until dishmachine is repaired and sanitizing properly. 0 ppm chlorine. 35A-05-4    High Priority - Roach activity present as evidenced by live roaches found. 1 live roach on cart holding bread in hallway near cook line. 6 live roaches in cove molding between two coolers on cook line. **Repeat Violation** **Warning**
