A summary of the violations found during the inspection are listed below.
The department cites violations of Florida's sanitation and safety laws,
which are based on the standards of U.S. Food and Drug Administration's Food Code.
High Priority violations are those which could contribute directly to a foodborne
illness or injury and include items such as cooking, reheating, cooling and hand-washing.
Intermediate violations are those which, if not addressed, could lead to risk factors
that contribute to foodborne illness or injury. These violations include personnel training,
documentation or record keeping and labeling. Basic violations are those which are considered
best practices to implement. While most establishments correct all violations in a timely
manner (often during the inspection), the division's procedures are designed to compel
compliance with all violations through follow-up visits, administration action or closure
when necessary.
Basic - Equipment in poor repair. Observed Raw salmon 47°, raw tilapia 47°, raw grouper 47°, raw tuna 48° cold hold in the Frigidaire reach in cooler in Cookline.
Basic - Potentially hazardous (time/temperature control for safety) food thawed in standing water. Observed fish thawing at room temperature in the 3 compartments sink.
Basic - Wall not smooth, nonabsorbent and easily cleanable in food preparation, food storage, warewashing areas or bathroom, in restrooms and dry storage area.
High Priority - Clams/mussels/oysters cold held at a temperature greater than 41 degrees Fahrenheit. Observed Raw salmon 47°, raw tilapia 47°, raw grouper 47°, raw tuna 48° cold hold in the Frigidaire reach in cooler in Cookline.
High Priority - Roach activity present as evidenced by live roaches found. 10 live roaches behind the reach in freezer in the dry storage area, 4 live roaches under the shelf of the dry storage area, 5 live roaches behind the Frigidaire reach in cooler in the hallway, observed 10 live roaches under the prep table in the kitchen area by the Frigidaire reach in cooler, 3 live roaches behind the Frigidaire reach in cooler at front counter.
High Priority - Rodent activity present as evidenced by rodent droppings found. Observed approximately 80 fresh droppings under the shelves of the dry storage area.
Intermediate - Food manager certification expired. A list of accredited food manager certification examination providers can be found at http://www.myfloridalicense.com/dbpr/hr/food-lodging/managercertification.html.
