A summary of the violations found during the inspection are listed below.
The department cites violations of Florida's sanitation and safety laws,
which are based on the standards of U.S. Food and Drug Administration's Food Code.
High Priority violations are those which could contribute directly to a foodborne
illness or injury and include items such as cooking, reheating, cooling and hand-washing.
Intermediate violations are those which, if not addressed, could lead to risk factors
that contribute to foodborne illness or injury. These violations include personnel training,
documentation or record keeping and labeling. Basic violations are those which are considered
best practices to implement. While most establishments correct all violations in a timely
manner (often during the inspection), the division's procedures are designed to compel
compliance with all violations through follow-up visits, administration action or closure
when necessary.
Basic - Case/container/bag of food stored on floor in walk-in cooler/freezer: case on onions on walk-in cooler floor. Observed operator remove food off floor and store properly. **Corrected On-Site** **Warning**
Basic - Employee beverage container on a food preparation table or over/next to clean equipment/utensils: drink over prep table.

Basic - In-use utensil not stored with handle above the top of potentially hazardous (time/temperature control for safety) food and rim of the container: spoon handle touching taziki sauce.

High Priority - Live, small flying insects in kitchen, food preparation area, or food storage area: Observed two on handle at three compartment sink, one on dry storage shelf and one on shelf next to reach-in freezer. **Repeat Violation** **Admin Complaint**
High Priority - Raw animal food stored over ready-to-eat food in reach-in cooler: raw chicken, beef and steak over cooked gyro, ham and beef.

High Priority - Rodent activity present as evidenced by rodent droppings found: approximately 50-100 droppings on the floor underneath grill: observed some hard droppings and due to the location of the droppings, can not determine whether the droppings are fresh or not. **Warning**
High Priority - Toxic substance/chemical stored by or with clean or in-use utensils: sanitizing tablet next to clean cups by three compartment sink at bar.

Intermediate - Interior of reach-in cooler soiled with accumulation of food residue: both reach-in coolers in the kitchen and vegetable chopper/slicer.

Intermediate - Manager lacking proof of food manager certification. A list of accredited food manager certification examination providers can be found at http://www.myfloridalicense.com/dbpr/hr/food-lodging/managercertification.html. This violation must be complied by 1/5/18. **Warning**
Intermediate - No proof of required state approved employee training provided for any employees. To order approved program food safety material, call DBPR contracted provider: Florida Restaurant and Lodging Association (SafeStaff) 866-372-7233.
