A summary of the violations found during the inspection are listed below.
The department cites violations of Florida's sanitation and safety laws,
which are based on the standards of U.S. Food and Drug Administration's Food Code.
High Priority violations are those which could contribute directly to a foodborne
illness or injury and include items such as cooking, reheating, cooling and hand-washing.
Intermediate violations are those which, if not addressed, could lead to risk factors
that contribute to foodborne illness or injury. These violations include personnel training,
documentation or record keeping and labeling. Basic violations are those which are considered
best practices to implement. While most establishments correct all violations in a timely
manner (often during the inspection), the division's procedures are designed to compel
compliance with all violations through follow-up visits, administration action or closure
when necessary.
Basic - Dead roaches on premises. 2 dead roaches behind sushi bar under hand wash sink, 2 dead roaches under bag in box cola syrup rack, 2 dead roaches in dining room. **Warning**
High Priority - Employee switched from working with raw food to ready-to-eat food without washing hands. Employee behind sushi bar touched raw fish and than touched tempura shrimp. **Warning**
High Priority - Nonexempt fish offered raw or undercooked has not undergone proper parasite destruction. Fish must be fully cooked or discarded. Invoice states the salmon is aquaculture but does not state pellet fed. Operator stated they are performing parasite destruction in the establishment but could not provide proof of dates or temperature. Educated operator on proper documentation needed for parasite destruction **Warning**
High Priority - Ready-to-eat, potentially hazardous (time/temperature control for safety) food marked with a date that exceeds 7 days after opening/preparation. **Corrected On-Site** **Warning**
High Priority - Roach activity present as evidenced by live roaches found. 5 live roaches behind sushi bar. 2 live roaches on wall behind co2 tanks near iced tea machines, 2 live roaches under ice cream freezer. 1 live roaches on wall near bags of rice in dry storage area. **Warning**
High Priority - Toxic substance/chemical stored by or with clean or in-use utensils. Pesticides dispensed onto the walls in cabinets holding utensils not protected.. **Corrective Action Taken** **Warning**
Intermediate - Ready-to-eat, potentially hazardous (time/temperature control for safety) food prepared onsite and held more than 24 hours not properly date marked. **Corrected On-Site** **Warning**
