Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

36-66-4    Basic - Ventilation inadequate as evidenced by smoke in kitchen area. **Warning**
02D-01-5    Basic - Working containers of food removed from original container not identified by common name. Observed a large container of salt is not labeled. **Warning**
03B-02-4    High Priority - Cooked meats or poultry hot held at less than 135 degrees Fahrenheit or above. Observed cooked chicken left at room temperature in the kitchen. The chicken was 83-99F. The chicken was put on a grill to be reheated. **Corrective Action Taken** **Warning**
12A-07-4    High Priority - Employee failed to wash hands before changing gloves and/or putting on gloves to work with food. Observed cook handle raw fish and then put on gloves without washing hands. **Warning**
12A-10-4    High Priority - Employee touched bare body part and then engaged in food preparation, handled clean equipment or utensils, or touched unwrapped single-service items without washing hands. Observed employee taste rice with a bare hand and continue cooking without washing hands . Mgr corrected the employee and he properly washed hands. **Corrected On-Site** **Warning**
12A-20-4    High Priority - Employee washed hands with no soap. Observed Cooke washing hands with no soap at the 3 compartment sink. He properly rewashed hands at a hand sink. **Warning**
03D-05-4    High Priority - Potentially hazardous (time/temperature control for safety) food cooked/heated yesterday/on a previous day not cooled from 135 degrees Fahrenheit to 41 degrees Fahrenheit within a total of 6 hours. Food has been cooling overnight. See stop sale. Observed cooked red beans prepared on 12/27/17 are still 45-46F covered in a pan approximately 7 deep. **Warning**
03B-01-4    High Priority - Potentially hazardous (time/temperature control for safety) hot held at less than 135 degrees Fahrenheit or above. Observed multiple cooked foods in the front counter steam table are below 135F. These pans of food are too overloaded to keep the food at proper temperatures. Also observed fried rice in a rice warmer at 108F. **Warning**
41-02-4    High Priority - Toxic substance/chemical stored by or with food. Observed a container of soy sauce stored by bleach and soap under the hand sink by the 3 compartment sink. **Warning**
41-18-4    High Priority - Warewashing sanitizing solution exceeding the maximum concentration allowed. Observed 2 chlorine sanitizer pails at well over 200 ppm when tested. **Warning**
41-17-4    Intermediate - 2 Spray bottles containing toxic substance not labeled above the mop sink. **Warning**
53B-12-4    Intermediate - Employee trained using a state approved in-house training program not performing their work duties in a manner consistent with the Food Code requirements as evidenced by the number of violations. **Warning**
31A-13-4    Intermediate - Employee used handwash sink as a dump sink. Observed the manager dump ice into the hand sink behind the front counter. **Warning**
12A-03-4    Intermediate - Employee washed hands in a sink other than an approved handwash sink. Observed Cooke washing hands with no soap at the 3 compartment sink. He properly rewashed hands at a hand sink. **Corrected On-Site** **Warning**
22-22-4    Intermediate - Encrusted material on can opener blade. **Warning**
22-11-4    Intermediate - Food-contact surfaces in contact with potentially hazardous (time/temperature control for safety) food not cleaned at least every four hours. Cook stated that a pan used to flour raw fish is only cleaned daily. **Warning**
31A-03-4    Intermediate - Handwash sink not accessible for employee use due to items stored in the sink next to the 3 compartment sink. The lid was removed. **Corrected On-Site** **Repeat Violation** **Warning**
53A-05-5    Intermediate - No currently certified food service manager on duty with four or more employees engaged in food preparation/handling.
