Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

38-07-4    Basic - Lights in food preparation, food storage or warewashing area missing the proper shield, sleeve coatings or covers. In the kitchen area.
14-67-4    Basic - Reach-in cooler gasket torn/in disrepair. In Chest reach in freezer closest to door next to.
36-27-5    Basic - Wall soiled with accumulated grease, food debris, and/or dust. Behind stove and fryer in the kitchen.
42-01-4    Basic - Wet mop not stored in a manner to allow the mop to dry. **Repeat Violation**
21-02-4    Basic - Wet wiping cloth used to wipe tableware or carry-out containers. On preparation table.
50-17-2    High Priority - Operating with an expired Division of Hotels and Restaurants license. Observed seating license for the establishment from the Division of Hotels and Restaurant expired on October 1, 2017.
03A-02-4    High Priority - Potentially hazardous (time/temperature control for safety) food cold held at greater than 41 degrees Fahrenheit. 35A-05-4    High Priority - Roach activity present as evidenced by live roaches found. Observed approximately 15 live roaches crawling throughout the preparation table in the kitchen.
01B-02-4    High Priority - Stop Sale issued on potentially hazardous (time/temperature control for safety) food due to temperature abuse. Cooked rice (57°F - Cold Holding); cooked rice (57°F - Cold Holding); cooked yellow rice (61°F - Cold Holding). As per operator food was placed in reach in cooler approximately at 3:00 pm from day before.
22-25-4    Intermediate - Accumulation of encrusted food debris on/around mixer head. Next to reach in cooler near three compartment sink.
31A-04-4    Intermediate - Handwash sink removed from food preparation/dishwashing area. Must be reinstalled in the same location where removed. Observed hand wash sink sign, paper towel and soap dispenser as well as broken sink on top of kitchen equipment, were hand wash sink was removed.
16-35-4    Intermediate - No chlorine chemical test kit provided when using chlorine sanitizer at three-compartment sink/warewashing machine. **Repeat Violation**

14-77-6    Intermediate - Reach-in cooler not maintained in good repair. Do not store potentially hazardous (time/temperature control for safety) food in this unit until the unit is repaired. Door handle Tropicana cooler in the kitchen, preventing door for been closed. **Repeat Violation**
02C-02-4    Intermediate - Ready-to-eat, potentially hazardous (time/temperature control for safety) food prepared onsite and held more than 24 hours not properly date marked. Cooked rice, cooked chicken in the reach in cooler near three compartment sink. **Repeat Violation**
53B-10-4    Intermediate - Records/documents for required employee training do not contain all of the required information. 2 Certification missing date of completion and expiration.
