Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

Observed employees phone and other personal devices stored on shelf with single service cups and lids in dry storage area. Operator removed personal items and properly stored. **Corrected On-Site**

21-08-4    Basic - Wiping cloth quaternary ammonium compound sanitizing solution not at proper minimum strength. Observed 0 ppm when tested. Operator replaced with chlorine sanitizer at 100 ppm when tested. **Corrected On-Site**
01B-37-4    High Priority - Cooked/heated potentially hazardous (time/temperature control for safety) food not cooled from 135 degrees Fahrenheit to 41 degrees Fahrenheit within six hours. See stop sale. Observed Cooked beans 45 degrees F, cooked beef 44 degrees F, cooked chicken 44 degrees F. Observed raw shrimps in reachin cooler over night at 48 degrees F **Repeat Violation**
12A-19-4    High Priority - Employee washed hands with cold water. Observed employees washed hands with hot water in dishwashing area. No hot water observed at handwash sink in dishwashing area.
35A-02-5    High Priority - Live, small flying insects in kitchen, food preparation area, or food storage area. Observed 3 live flies flying around in dishwashing and ceviche prep area and landing on prep table. **Admin Complaint**
03A-02-4    High Priority - Potentially hazardous (time/temperature control for safety) food cold held at greater than 41 degrees Fahrenheit. Raw shrimps 48 degrees F in reachin cooler overnight.
03D-05-4    High Priority - Potentially hazardous (time/temperature control for safety) food cooked/heated yesterday/on a previous day not cooled from 135 degrees Fahrenheit to 41 degrees Fahrenheit within a total of 6 hours. Food has been cooling overnight. See stop sale. Observed cooked beans dated 3/17 in walk-in cooler at 45 degrees F, cooked chicken dated 3/17 at 44 degrees F, cooked beef dated 3/17 at 44 degrees in walk-in cooler. **Repeat Violation**
03G-11-4    High Priority - Reduced oxygen packaged fish packaged onsite no longer frozen and not removed from reduced oxygen package. Observed talapia thawing in reachin cooler in ROP package.
35A-05-4    High Priority - Roach activity present as evidenced by live roaches found. Observed 1 live in inoperable reachin cooler next to ceviche prep area. **Admin Complaint**
29-42-4    High Priority - Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet.
22-20-4    Intermediate - Accumulation of black/green mold-like substance in the interior of the ice machine.
27-16-4    Intermediate - Hot water not provided/shut off at employee handwash sink. Observed no hot water at handwash sink in dishwashing area.
14-16-4    Intermediate - Multiuse food-contact surface not smooth, free of breaks/cracks/chips/pits/crevices. Observed plates with edges cracked in dishwashing area.
16-34-4    Intermediate - No quaternary ammonium chemical test kit provided when using quaternary ammonium chemical sanitizer at three-compartment sink/warewashing machine. None observed to test quaternary concentration.
41-17-4    Intermediate - Spray bottle containing toxic substance not labeled. Observed next to 3 compartments sink.
