Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

36-37-4    Basic - Ceiling not smooth, nonabsorbent and easily cleanable in food preparation, food storage, warewashing areas or bathroom. Observed ceiling tiles in food prep area non absorbent and not easily cleanable **Warning**
14-46-4    Basic - Shelves not attached to walls in walk-ins not constructed of materials that are corrosion resistant, non-absorbent, smooth and easily cleanable. Observed rusty shelves in the walk-in cooler. **Warning**
29-20-5    Basic - Standing water or very slow draining water in handwash sink, Observed stagnant water in the wash sink next to three compartment ware washing sink . **Repeat Violation** **Warning**
14-41-4    Basic - Walk-in cooler gasket torn/in disrepair. Observed water dripping from the top of the walk-in cooler due gaskets in disrepair. **Warning**
09-01-4    High Priority - Employee touching ready-to-eat food with their bare hands - food was not being heated as a sole ingredient to 145 degrees F or immediately added to other ingredients to be cooked/heated to the minimum required temperature to allow bare hand contact. Establishment has no approved Alternative Operating Procedure. Observed employee touch cut tomatoes in the flip top cooler with bare hands. **Warning**
03A-02-4    High Priority - Potentially hazardous (time/temperature control for safety) food cold held at greater than 41 degrees Fahrenheit. Observed cooked potatoes (44-45°F - Cold Holding) in the walk-in cooler, item was placed in walk in cooler from yesterday. . See stop sale . **Warning**
08A-01-4    High Priority - Raw animal food stored over cooked food. 1. Observed raw beef stored over cooked rice in the reach-in cooler on the cookline. Operator discarded the cooked rice. See stop sale *corrected on-site**. 2. Observed raw beef stored over cooked rice in the walk-in in cooler, and also bottles of sauces stored over cooked rice. 3. Observed raw shell eggs stored over cooked beans In the walk-in cooler, 4. Observed boxes with duck legs stored over fish filet in the walk-in cooler . 5. Observed raw chicken stored over fish in the walk-in cooler. 6. Observed raw beef stored over Tamale in the reach-in cooler at entrance to kitchen. Operator removed tamale and placed it correctly. **Corrected on site**. **Warning**
08A-02-5    High Priority - Raw animal food stored over or with ready-to-eat food in walk-in freezer - not all products commercially packaged. Observed raw beef on top cooked chicken in reach -in freezer next to the dry storage room. Operator removed the raw beef and placed it correctly. **Corrected On-Site** **Warning**
01B-02-4    High Priority - Stop Sale issued on potentially hazardous (time/temperature control for safety) food due to temperature abuse. 29-34-4    High Priority - Vacuum breaker missing at hose bibb or on fitting/splitter added to hose bibb. Observed that there is no vacuum breaker attached to hose bibb located at the handwash sink next to three compartment warewashing sink. **Repeat Violation** **Warning**
11-03-4    Intermediate - Certified Food Manager or person in charge lacks knowledge of employee health policy and health requirements. **Repeat Violation** **Warning**
12A-03-4    Intermediate - Employee washed hands in a sink other than an approved handwash sink. Observed one of the employees wash hands in the ware washing sink in the food preparation area. 03D-15-4    Intermediate - Food being cooled by nonapproved method as evidenced by inadequate rate of cooling during time of inspection. Observed rice at temperature 44°F and Potatoes at temperature 44-45°F in the walk-in cooler, using unapproved method as noticed by condensation in plastic covering . 02C-02-4    Intermediate - Ready-to-eat, potentially hazardous (time/temperature control for safety) food prepared onsite and held more than 24 hours not properly date marked. Observed cooked rice in in the flip top cooler not date marked, as per operator the rice was cooked on the 05/19/2019. Operator was instructed to date mark the rice. Corrected on site. Observed cooked rice in walk-in cooler not date marked, as per operator the rice was cooked 05/19/2019. Observed cooked pasta, potatoes and cut tomatoes in walk in cooler not date marked. **Repeat Violation** **Warning**
