Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

32-02-4    Basic - Bathroom door left open other than during cleaning or maintenance.

14-01-4    Basic - Bowl or other container with no handle used to dispense food. Plastic container without handle to dispense beans.
23-06-4    Basic - Build-up of food debris, dust or dirt on nonfood-contact surface. Observed greasy and dirty shelving, exterior of cooking equipment, walking cooler shelving . **Repeat Violation**
23-25-4    Basic - Build-up of grease/dust/debris on hood filters.
08B-44-4    Basic - Case/container/bag of food stored on floor in kitchen. Observed onion bag on the kitchen floor. Employee moved it immediately to a shelf. **Corrected On-Site**
36-34-5    Basic - Ceiling/ceiling tiles/vents soiled with accumulated food debris, grease, dust, or mold-like substance. Observed accumulation of grease and dust on ceiling vent in kitchen area.
14-09-4    Basic - Cutting board has cut marks and is no longer cleanable.
40-06-4    Basic - Employee personal items stored in or above a food preparation area. Observed employee personal bag and cell phones on preparation table.
51-09-4    Basic - Establishment did not report seating change that affects the license fee, Clean Indoor Air Act, sewage system approval or other related requirements. According owner they have been approved for 32 indoor seating only with Derm. Observed 16 indoor seats at the time of the inspection.
29-09-4    Basic - Faucet/handle missing at plumbing fixture. Observed a missing faucet ( hot line faucet) in one of the compartment at 3 compartment sink in the kitchen.
36-18-4    Basic - Floor tiles cracked, broken or in disrepair. **Repeat Violation**
36-14-4    Basic - Grease accumulated on kitchen floor and/or under cooking equipment. Excessive accumulation of grease all throughout the kitchen floor , shelves and equipments in kitchen area. **Repeat Violation**
22-19-4    Basic - Interior of microwave soiled with encrusted food debris. Observed accumulation of old food debris inside microwave.
38-07-4    Basic - Lights in food preparation, food storage or warewashing area missing the proper shield, sleeve coatings or covers. Missing light cover in the kitchen.
05-17-4    Basic - No conspicuously located ambient air temperature thermometer in walk-in cooler. **Repeat Violation**
06-04-4    Basic - Potentially hazardous (time/temperature control for safety) food thawed at room temperature. Observed frozen pork left at room temperature to thaw ( for approximately 15 minutes according the owner). The owner placed the food under running water. **Corrective Action Taken**
36-27-5    Basic - Wall soiled with accumulated grease, food debris, and/or dust. Accumulation of grease , dust and food resin kitchen wall ( all throughout the area).
41-02-4    High Priority - Toxic substance/chemical stored by or with food. Observed degrease containers near onion bag under shelving on kitchen floor.. Owner moved the onion bag. **Corrected On-Site**
16-33-4    Intermediate - Chemical test kit not used to ensure proper sanitization of equipment and utensils when using a chemical sanitizer. As evidence of test kit inside sealed and unused packaged .
27-16-4    Intermediate - Hot water not provided/shut off at employee handwash sink. Hot line for running water was closed. Person in charge opened the hot line. **Corrected On-Site**
31B-05-4    Intermediate - Paper towel dispenser at handwash sink not working/unable to dispense paper towels. Handwash sink in the kitchen area.
22-16-4    Basic - Intermediate - Reach-in cooler shelves soiled with food debris. Empty reach in cooler with moldy shelves. Employee started cleaning. Reach in cooler was not in use at the time of the inspection. **Corrective Action Taken** **Repeat Violation**
22-18-4    Intermediate - Soil residue in food storage containers. Observed old food debris in food storage containers stored underneath the steam table.
